Job summary

An exciting opportunity to join the Teesside Enhancing Physical Health Team as a Senior Registered Nurse / Health Facilitation.

The team is currently in the process of merging with the Durham and Darlington Health facilitation team and will be the Durham Tees Valley Adult Learning Disability Physical Health Team.

The post holder will work as part of the Physical Health Teams, working to break down barriers to improve health outcomes and improve quality of life for people with a learning disability.

The position will provide high quality assessment to young adults with a learning disability to improve outcomes through transitions. You should be enthusiastic and show an understanding of the complex needs of people with a learning disabilities

The service is currently 9am to 5pm Monday to Friday.The post holder will also be part of the Nurse On Call rota that covers out of hours 5pm to 9am.

Main duties of the job

  • The post holder will work in the DTV ALD Physical Health Team with a focus on the additional needs of young people moving through transitions.
  • The post holder will be expected to manage a caseload, working autonomously with a broad spectrum of physical and mental health issues
  • The post holder will be expected to support with additional functions of the team including STOMP, EHCH, Health Facilitation of adults and the Epilepsy Nurse as and when required.
  • The post holder will support with students and junior staff as required and will understands

Qualifications

Essential

  • Current professional registration with Nursing & Midwifery Council - RNMH or RNLD
  • Evidence of Continuing Professional Development
  • Recognised Practice Supervisor and/or Practice Assessor, clinical educator or equivalent. Must be achieved within 6 months.
  • Trained in Clinical Supervision and providing clinical supervision OR willingness to work towards within 6 months

Experience

Essential

  • 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework)
  • Working collaboratively with service users and their families/carers
  • Providing clinical supervision to individuals or groups as professionally appropriate.
  • Working in a multi-disciplinary team.
  • Quality improvement activities.
  • Supervising or assessing students on practice placement

Knowledge

Essential

  • Detailed understanding of Safeguarding and its application in practice
  • Clinical Governance and its application in practice.
  • Clinical Risk Assessment and Management and its application in practice.
  • Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act)
  • Research and development methodology
  • Understanding of psychological models of care and treatment.
  • Working knowledge of clinical medicines management, including administration of injections
