An exciting opportunity has arisen for an enthusiastic individual with relevant experience of:

  • Implementing and delivering transformation and change management within and across a health and care system.
  • Robust system leading and effective partnership working
  • Using the principles of the commissioning cycle and evidence-based working in transformation work to join the Barnsley Place Transformation and Delivery Team of the ICB.

The Transformation and Delivery Team in Barnsley supports the ICB and partners to deliver our ambitions as set out within the Barnsley Health and Care Plan alongside the broader ambitions of South Yorkshire ICB.

The Senior Transformation and Delivery Manager will work as part of the team, taking the lead on developing and delivering effective change management, horizon-scanning to spot innovation opportunities to transform and improve health and care services for the population of Barnsley. The role will have a particular focus on prevention and management of long-term conditions, cancer and end of life and planned care.

Main duties of the job

The post holder will contribute to the development of South Yorkshire and Barnsley Place Transformation and Delivery functions. This will transform the work we undertake and will shape how we operate and align our capacity across the ICB to deliver priority transformation and change programmes.

The post holder will lead the delivery of specific work streams and programmes in relation health and care transformation and delivery.

Key work areas include:

  • Transformation work streams across ICB Place
  • Long Term Conditions, Prevention, Management and pathways/service redesign
  • Anticipatory/Proactive Care. Working with Primary Care and other place partners
  • Contributing to the development and delivery of plans that address health inequalities.
  • Planned care, elective and diagnostics, including referral management.
  • Cancer improvement and end of life care.
  • Contributing to the South Yorkshire System Control Centre arrangements and management of place based escalation arrangements.
  • Supporting the T&D team with Quality, Service Improvement and Redesign (QSIR)

Job responsibilities

Communication and relationships

  • Supporting collaborative arrangements and negotiations with providers, as part of the transformation, delivery and improvement function
  • Provide additional support to the wider transformation, delivery and improvement team through working flexibly to directly assist other portfolios of work where they may be a subject matter expert or through research and development and delivery of materials as required.
  • Provide expertise of best practice change and project management methodologies.
  • Communicate, engage and build relationships with key partners across a wide range of stakeholders including providers, other health and social care organisations (Councils) and commissioners.
  • Engage and manage relationships with stakeholders to achieve a common purpose and realise mutual benefits where there may be contentious decisions to be made around performance or finance etc.
  • Produce material for and present complex presentations to large groups which are engaging, influential and effective in delivering key messages, the content of which may include highly complex technical, contentious or very sensitive information.
  • Forge close working relationships across the organisation to facilitate an effective matrix style approach to delivery of contracts, services, projects and programmes of work.

Analytical and judgement

  • Identify options by utilising specialist knowledge using agreed criteria and initiative to test solutions in advance.
  • Contribute to the development of key performance indicators and milestones for specific projects/services.
  • Interrogate and analyse, sometimes highly complex, information to support the delivery of clear reporting and outcomes.
  • Interpret national policy and guidance in relation to specific areas of work to support the delivery of local strategy and plans.
  • Write and submit regular reports for senior managers and executives showing progress against milestones, status, resource requirements, issues, risks and dependencies, making recommendations where a range of options may exist.

Planning and organisation

  • Managing the set-up and delivery of services and projects, working with colleagues and subject matter experts on the strategic planning, scoping, design and the delivery.
  • Deliver service change/redesign linked to requirements from NHS England and NHS Improvement which are complex in nature and that supports system transformation.
  • To support the development and implementation of new/existing services, including planning and needs assessment, development/review of service specifications, participating in the procurement process, issuing contractual documentation, supporting mobilisation and ongoing contract management including improvement and performance of services.
  • Manage the whole commissioning cycle, reporting on progress and managing risks and issues, escalating where necessary to the appropriate board.
  • Support organisational change and the uptake of initiatives that support excellence.
  • Ensure benefits are identified, understood, measured, tracked and owned.
  • Demonstrate flexibility and support new portfolios of work where necessary.

Policy and Service Development

  • To ensure all public contact with the team is of highest professional standard.
  • To embed patient and public involvement where appropriate at all levels of decision making.
  • Be a champion for best practice, learning from experience and from others, supporting the spread of innovation within the organisation.
  • Develop and implement standardised operation processes and procedures for projects, and work with senior team members to develop project management policies that apply across SYICB.
  • Keep patient interest at the centre of all activities and involve patients and public in policy development and service, project and programme delivery where appropriate.
  • Works within organisation and national guidance, interpreting national guidance relevant to the area of work and acting a subject matter specialist within the specific area.
  • Support the work at Place and across SYICB to ensure value for money across commissioned services including the delivery of any required efficiency/change programmes

Research and Development

  • Undertake research and development initiatives, delegating as appropriate.
  • Plan, develop and evaluate methods and processes for gathering, analysing, interpreting and presenting data and information.
