Job summary

The post holder will work with a team of Gastroenterologists, GPwERs and Health Care Assistants providing predominately face to face consultations, in one of our community based clinics. The clinics provide pre-booked appointments for adult patients referred to the service. A range of gastroenterology conditions will be seen in the clinics, including, but are not limited to, dyspepsia, abdominal pain, food intolerance, irritable bowel syndrome, constipation, NAFLD, raised Ferritin.We offer flexible working in a purpose built environment and are open 7 days per week, 8am-8pm. The post is offered on a Professional Services Agreement and sessions and hours are negotiable.

Main duties of the job

Participate with the team to optimise performance and ensure the delivery of high quality evidence based care.

Inspire and motivate the team members providing clear goals and recognising accomplishments

Offer support and mentorship, promoting good practice and a learning environment

Be a source of clinical expertise, facilitating multidisciplinary team meetings

Participate in clinical educational sessions and support the development of junior colleagues

Participate in clinical audit as per HH policy and procedure

Actively contribute to development and efficiency of the service

Be competent in managing common gastroenterology presentations

Optimise the medical management of this group of patients, in agreement with the patient and discharge them back to their registered GP

Refer to secondary care and / or other services as appropriate

Complete comprehensive, contemporaneous notes

Be a source of clinical expertise to less experienced staff, and work in partnership within a multi-disciplinary team as appropriate

Participate in clinical meetings, HH clinical governance agenda, training and education

Be responsible for any pathology or radiology results that are ordered by themselves and support team members in interpreting the same as needed.

About us

Harrow Health Community Interest Company (HH) began in 2007 by a group of Harrow GPs, whose sole aim was to ensure patients receive a high quality clinical care service within the community. Over the years HH has expanded. We now deliver a number of community services including ENT, Dermatology, Gynaecology, ADHD and Enhanced Frailty Services.

HH Community Services directorate has been awarded a contract to provide a general Gastroenterology service on behalf of NWL ICB. This is also a Right to Choose contract across the country.

Working in partnership with other stakeholders HH will continue to provide gold standard services and develop a sustainable future for the local health community.

Job responsibilities

Main Responsibilities

  • To support the Gastroenterology team to deliver a high standard of evidence based care to those attending the service
  • Participate in Multidisciplinary Team meetings
  • Work with the team to maintain a high quality, clinically effective and cost effective primary care service
  • To provide a comprehensive service to the patients attending the Gastroenterology clinic including clinical examination, investigations as needed, diagnosis and management, and referral as appropriate
  • To ensure patients have consented to, and are fully involved in the implementation of their procedure / treatment plans
  • Interpretation of results where requested for individuals
  • If a prescription is needed, this must be in line with the NHS England and local guidelines
  • To adapt styles of communication appropriately in order to promote empowerment, self-management and health education for patients. Interpreters can be sought as needed.
  • Using the EMIS system the consultation must be recorded in the patients records. Where the patient is registered with a Harrow GP the clinician will have access to their GP records. Proformas are set on the EMIS system and must be completed by the clinician. These will be shared with all the referring clinicians
  • Liaise with other team members as appropriate
  • To identify actual or potential clinical risks within scope of practice and take appropriate action using, when required, the appropriate reporting structure
  • To be personally accountable for professional and ethical actions and practices within a framework that adheres to the GMCs requirements and HH policies and procedures

Service delivery responsibilities:

  • To ensure that patient records and documentation are maintained in line with HH policies and use of the electronic patient records system. This includes:
  • Documenting full, accurate and contemporaneous records of assessments, investigations, procedures, treatments and aftercare.
  • Encouraging colleagues to maintain good and complete records.
  • Working closely with the reception team / administrator, ensuring all records pertinent to patients are filed as appropriate
  • Work within local policies, protocols and guidelines.
  • Ensure Safeguarding Adults referral is considered in cases of suspected abuse and policies and procedures are followed when concerns are raised
  • To participate in audits as requested, and use the results, current research, and government guidelines to make recommendations for changes to service delivery and clinical practice, sharing these findings as appropriate.
  • To collect data, and provide service information to the Service Manager as required.
  • To ensure that the Clinical Manager / Service Manager / Head of Nursing and Clinical Governance Lead are kept informed on service issues and potential risks.
  • To work with the Service Manager in delivering and promoting the needs of the service
  • To work with the Clinical Lead, Service Manager, Head of Nursing and Clinical Governance Lead to meet the Clinical Governance needs of the services, including investigation of incidents (including serious incidents) and complaints, where required.
  • To remain updated in the use all equipment used in the service and to report any technical problems or issues with that equipment.
  • To participate in team meetings to discuss day to day operational issues as required.

Personal & professional development:

  • To be a skilled practitioner in areas needed for effective care delivery
  • o Recognise and work within own competence and keep abreast of developments in practice and topical issues ensuring that skills are maintained
  • To collaborate in the continuing development of the Consultant role to include:-
  • o Level of autonomy in practice
  • o Show self-motivation toward expanding practice and developing skills and knowledge, with the support of HH
  • Ensure statutory and mandatory training is up to date

Policies & Guidelines:

  • Awareness of and compliance with all relevant company policies / guidelines, e.g. confidentiality, data protection, health and safety
  • To participate and contribute to operational policy making, attending appropriate meetings as required.
  • Work within policies relating to safeguarding, prevent and other HH Policies

  • The Post holder must be able to work independently within defined responsibilities, but also as part of a larger team and be able to trouble shoot and find solution to problems
  • To adhere to Information Governance, patient confidentiality and Data protection legislations/standards.
  • To work to standards set out in HH policies, service contracts and in line with NHS England/ CQC regulations
  • To support the team to ensure that all HH policies, standards and requirements are understood and in place in the service to in include excellent record keeping and delivery of high standard patient and customer services
  • To understand service specifications and contracts
  • Represent HH effectively and professionally to a wide range of patients, stakeholders, members and partners
  • Take an active role in the development and embedding of the organisations culture, values and reputation as a provider of high quality services, leading by example at all times
  • Responsible for policy implementation related to own Service area
  • Pro-actively look for growth opportunities

  • The post holder is expected to work with a high level of individual discretion and autonomy, knowing when to partner with colleagues as appropriate
