We are looking for a Lead Nurse for Single Point of Access to join the Dudley Clinical Hub to provide clinical, professional and managerial leadership. The successful candidate will be expected to deliver a high quality service to ensure all patients and clinicians can access the service to receive the support they require and by working clinically, as required by service demand, as a member of the team you will ensure a positive patient experience. You will be expected to coordinate and manage capacity on a daily basis to allow efficient patient flow and to help achieve the two hour urgent response time within the Triage- Clinical Hub working environment.

The Service will operate between the hours of 8am - 9pm, 7 days per week, and 365 days per year. The successful applicant will be expected to work flexibly throughout this period.

Main duties of the job

The Dudley Clinical Hub (DCH) provides a single point of access for patients registered with a Dudley GP for all admission avoidance referrals from professionals and for patients requiring Health and Social Care Services.We are looking for a registeredNurse who has demonstrable clinical post registration experience to support and manage our Triage team within the Dudley Clinical Hub.They will work alongside the Medical Lead, Operational Leads and Advanced Clinical Practitioners to empower our triage Nurses to support our service to give our patients the best experience possible.

Based at Brierley Hill Health and Social Care Centre, the successful applicant will join an integrated, collaboratively focused team, employing a proactive clinical practice and knowledge to provide phone triage / clinical assessment skills in order to support urgent care for patients across our health economy.The primary objectives of DCH triage and two hour urgent response team is to prevent unnecessary admission to acute care and to facilitate collaborative MDT management for patients closer to home.

We need the applicants to be flexible, approachable and have a sound clinical evidence based background and knowledge. They will need to be friendly but assertive, and be confident in their own decision making and be able to challenge when necessary.

Qualifications

Essential

  • First level registration
  • Evidence of undertaking a recognised course of study in teaching and assessing.
  • Educated to diploma level or can demonstrate experience of operating at this level
  • Evidence of professional development and regular study since qualification.

Desirable

  • Educated to first degree level or evidence of study at degree level.

knowledge and Training

Essential

  • Proven nursing / midwifery experience since qualification. Evidence of substantial Triage work and dealing with acutely unwell patients.

Experiance

Essential

  • Experience of regularly managing a team of staff and other resources in the acute/ community setting.
  • Experience of close liaison with members of multidisciplinary team.
  • Able to work flexibly e.g., support other Interview Nursing/AHP Teams/Services.

Communication and Relationship Skills

  • Ability to demonstrate negotiating and influencing skills
