Mocketts wood Surgery is seeking a dedicated, empathetic and passionate Social Prescribing Link Worker to join our team. We are looking for a proactive individual who is committed to understanding and supporting our patients. This role is motivated by the desire to help patients become active and independent members of their communities.

The ideal candidate will have experience in working positievley with pepole facing complex social and emotional challenges, as well as knowledge of solution focused coaching approaches. you will be working with people from diverse cultural and social backgrounds and must be able to work confidently and effectively.

At Mocketts Wood Surgery you will be identifiying local community resources and facilitate connections between these resources and the practice for the benefit of our patients. You will develop relationships and processes with organisations, health services and voluntary organisations.

We would like our Social Prescriber to empower individuals to take control of their health and wellbeing through referrals and support the growth of sustainable community groups. The role also involves working closely with the Primary Care network multi-disciplinary team.

Main duties of the job

Responsibilities of the role:

Provide personalised support to patients and their families, experiencing complex social circumstances, using person centred principles to build non-dependent, trusted relationships, for them to take control of their health and wellbeing. To make a positive difference to their lives.

Support access to community based services and activities that will help individuals live well, challenging service providers where necessary for the benefit of the patient.

Build effective pathways to support individuals and help them navigate their way through systems. Develop trusting relationships by giving people time and focus on "what matters to me". Take a holistic approach and co-design a simple personalised support plan to improve health and wellbeing.

Manage workload, appointments and priorities effectively and efficiently.

Maintain accurate records, deliver reports within timelines specified. Support the primary care team in building and maintain knowledge of local services and activities. Develop professional working relationships to ensure patients needs are met. Enable individual's to identify personal goals and connect with relevant services to address barriers to their health and wellbeing. Develop and deliver community groups and activities where appropriate. Comply with all policies and procedures related to the role and maintain flexibility to meet individual needs.

About us

Mocketts Wood Surgery is a GP Surgery looking after 9300 patients.

We are based in the beautiful village of St. Peters in Broadstairs. We currently have a fabulous team comprising of 5 GPs, 1 x Advanced Paramedic Practitioner, 2 x Practices Nurses, 1 nurse associate, 2 x Health Care Assistant, 1 x Trainee Nurse Associate, Frailty and Social Care Coordinator, pharmacist, and administrators, who work together to provide high levels of clinical excellence and support to our patients.

We are looking to complete and enhance our team by fulfilling the post of an additional friendly, positive and an enthusiastic Social Prescriber to deliver care to our patients.

Patient care is our priority and is at the centre of all that we do.

We therefore would expect the successful applicant to have a skill set that includes strong organisational and communication skills and most importantly to have empathy, be caring and thoughtful enough to go that extra mile to provide outstanding care to our patients.

Job responsibilities

Social Prescribing Link Worker

Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids.

Bring together all of a persons identified care and support needs, and explore their options to meet these into a single personalised care and support plan, in line with best practice.

Help people to manage their needs, answering their queries and supporting them to make appointments.

Support people to take up training and employment, and to access appropriate benefits where eligible.

Raise awareness of shared decision making and decision support tools, and assist people to be more prepared to have a shared decision making conversation.

Ensure that people have good quality information to help them make choices about their care,

Support people to understand their level of knowledge, skills and confidence (their Activation level) when engaging with their health and wellbeing, including through use of the Patient Activation Measure.

Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing.

Explore and assist people to access personal health budgets where appropriate.

Provide coordination and navigation for people and their carers across health and care services, alongside working closely with the 3rd sector within the community, health and wellbeing coaches and other primary care roles.

Demonstrate flexibility and adaptability to working in a dynamic environment.

To liaise and work with multiple services and external stakeholders, acting as a conduit for information sharing and communication between the ICB, GPs, Practices, KCHFT, Social Care Teams, KCC Commissioners, patients and voluntary sector.

Have a sound knowledge of health and social care policy, together with local services and health promotion initiatives, that will enable the safe transfer of clients between different provider services, and the integration of services from different providers where indicated

Support the continued development and coordination of an Integrated Health and Social Care Multi Disciplinary team meetings approach across the Primary Care Network. This will provide a coordinated response to referrals from GPs and health and social care teams.

The Social Prescribing role is critical in ensuring that patients are signposted to the correct health, social or voluntary agency in a timely manner.

Qualifications

Essential

  • COMMUNICATION AND RELATIONSHIP SKILLS:
  • To provide a single point of contact for GPs
  • to support them with investigating service user/patient case history to improve coordination of care
  • To coordinate and attend Inter-professional meetings, providing appropriate feedback.
  • To take Health and Social Care referral information according to required process on a daily basis. Ensuring accuracy and demonstrating non-judgmental and objective work practice and consideration of service users and carers views.
  • Develop and maintain effective working relationships with integrated teams including long term and practice linked teams, GP practices and other agencies to ensure that service users receive a consistent, integrated response to all contacts/referrals.
  • To have advanced communication skills, being able to discuss difficult and possibly contentious issues with patients, relatives and health professionals.
  • Demonstrate an ability to undertake duties in an autonomous manner with advice from the professional and service lead within the scope of individual competence as appropriate.
  • To work with the Surgery team and others in determining the most appropriate response to individual clients in a crisis situation and in arranging and coordinating that response.
  • To analyse the information provided and determine which service best meets the need of the patient.
  • To have a clear concise escalation process to follow for acute responses as necessary.
  • PLANNING AND ORGANISATIONAL SKILLS:
  • To be able to plan, organise and prioritise a busy caseload with conflicting demands on time.
