Job summary

Manx Care is offering an exciting opportunity for an experienced Social Worker to join our Integrated Hospital Social Work Team on the beautiful Isle of Man. Were dedicated to shifting care from hospital to home, as we know recovery is better when patients are in their familiar surroundings.

Main duties of the job

In this role, youll work closely with patients, families, and healthcare providers to assess needs, plan care, and ensure smooth transitions from hospital to home. Youll play a key part in improving the discharge process for patients with complex needs, helping them access the right services for a successful recovery.   • Diploma in Social Work, Dip. SW,
  • CQSW).

Experience

  • Sufficient relevant experience as a
  • social worker.
