Job summary
An excellent opportunity has arisen for Ash Trees Surgery to recruit a diabetic specialist nurse, who will support our current team. The post holder will be a highly experienced Practice Nurse/Long-term conditions nurse and have a good knowledge of monitoring people with diabetes and titrating medications as required. Working with the medication management team and GPs to change medications as needed.
In this role you will be working as part of multi-disciplinary team to ensure that all sites are working consistently towards QoF targets, whilst reaching the desired timescales.
As an autonomous practitioner you will be able to demonstrate critical thinking and decision-making relating to long term conditions.

Main duties of the job
Direct clinical involvement in monitoring and management of diabetes.
Delivery of nursing care to patients in the practice population.
Providing advice, support and liaison with other practice staff.
Involvement in QOF management.

Job responsibilities
Specialist knowledge of managing diabetes.
Enable individuals with long term conditions and their carers to make informed choices concerning their health and wellbeing and to organise their own support, assistance and action and promoting self-care.
Support individuals with long term conditions and their carers to change their behaviour and to reduce the risk of complications.
Manage programmes of care for patients with acute and chronic disease by planning and evaluating care.
Work across boundaries and in partnership with primary and secondary care clinicians and social services, coordinating care promoting a multi-disciplinary approach.
Review prescribed medication and provide advice on all aspects of medicines management, within scope of practice. Refer to relevant clinicians where appropriate.

Qualifications
Essential
- RGN.
- Experience of monitoring and helping people manage their diabetes.
- A good knowledge of Nice guidance
Desirable
- Diplomas/ courses in care of patients with Diabetes.
- Non-medical prescribing qualification.
- Experience of titrating medications for diabetes.

- Experience of working with patients to achieve goals specific to their health needs.
- Experience in helping patients to manage their long-term conditions, specifically Diabetes.
Desirable
- Experience in working in Primary care.
