An exciting opportunity has arisen for a Band 6 Occupational Therapist to work with Halton Intermediate Care and Frailty Service. We are looking for an experienced and well organised individual with a passion for promoting independence, driving a strength-based approach and providing therapy interventions in a person's own home or discharge to assess unit in line with a home first approach.

We have been on an exciting journey to transform the intermediate care services within Halton and have brought together health and social care into a unified integrated team.   • Single Point of Access
  • Community Rapid Response / Frailty Team
  • Reablement Team
  • Intermediate Care Unit

You will work as part of a large multidisciplinary team that includes: GP's, Advanced Nurse Practitioners, Physiotherapists, Occupational Therapists, Social Workers, Therapy and Nursing Assistants, Dietician, Care Support Workers and Administrators.

The HICAF Service operates a 7 day service, currently covering the hours of 8am to 8pm, including bank holidays and weekends. You will be expected to work shifts to cover some of the weekends and bank holiday.

Main duties of the job

To help us achieve this, we expect you to hold responsibility for your own caseload, perform patient therapy assessments, provide a diagnosis, develop, implement and evaluate individualised holistic treatment programmes, and deliver a high standard of evidence-based care.

In all areas, you will work in a multi-disciplinary team, follow appropriate pathways of care, liaising with and referring to other agencies as appropriate.

We expect you to be hardworking, motivated with excellent clinical, communication, organisational and time management skills. A flexible approach to day-to-day tasks is essential, in order to meet the demands of the service. You will also be expected to travel across Halton, in line with service needs, so access to transport is essential

As a Band 6 Occupational Therapist, you will contribute to the clinical supervision of students and junior staff. You will participate in service improvement projects, audits, in service training, and work collaboratively with our hospital and community colleagues to ensure our patients receive outstanding care.

Qualifications

Essential

  • Under Graduate Degree in Occupational Therapy
  • HCPC Registration
  • Completion of relevant Post Graduate training

Desirable

  • Post registration qualification in specialist area
  • Clinical educators certification

Experience

Essential

  • Previous NHS experience
  • Experience of Band 5 /6 core rotations
  • Experience of supervising, appraising and training staff and students.
  • Experience of managing challenging situations

Desirable

  • Band 5/6 experience within Intermediate Care or related community role
  • Experience of audits, service development and implementation of change

Knowledge

Essential

  • Understanding of local and national drivers for service delivery
  • Awareness of trust core values
  • Continuing commitment to CPD
  • IT literate in word processing and spread sheets

Desirable

  • Awareness of clinical governance and its impact on therapy services
  • Participation in staff management

Skills

Essential

  • Able to use initiative and be proactive
  • Demonstrates being flexible within a team
  • Experience of effective contribution to MDT working
  • Demonstrate effective communications skills both written and verbal

  • Experience of single handed care equipment and approaches
