An exciting opportunity has arisen for an enthusiastic, well-motivated and dedicated Physiotherapist to join our Recovery At Home Team, South Tyneside.
This is a unique and rewarding role which will allow you to work with, lead and support colleagues during a time of innovation and development as our team focusses on the implementation of Urgent Community response and of Virtual Wards.
You will work within Directorate One of Community Services, supported by senior colleagues who are passionate and proud of the services we deliver to our community of South Tyneside and Sunderland.

This vacancy is for one full time post (37.5 hours per week) and one part time post (19 hours per week).
Main duties of the job
The team you will work with consists of an extensive MDT which includes nurse practitioners, occupational therapists, physiotherapists, health care assistants, therapy assistant/exercise practitioners and administrative staff. The team works closely alongside partner agencies including those from the local and voluntary sector. This is a significant strength to our team we are proud of and supports patient centred, integrated working.
The team delivers planned and unplanned care to people in their own homes, to assist in preventing hospital admission and supporting those who have recently been discharged from hospital, require enhanced intervention and short term rehabilitation support for their physical and functional health needs to remain at home. The service also covers rotation into Borrowdale House, a reablement unit.
The service is fast paced and you would be expected to see a wide range of patients who present with a variety of conditions when supporting the team clinically.
The service is committed to providing a sound environment for education and training to support the successful candidate within this role.
This role would involve working predominantly core hours of Monday to Sunday 08:00 am 20:00 pm with flexibility to the needs of the service. We offer flexible working within our service with a variety of shift patterns and flexibility to work until 8pm and to cover weekends.
Candidates must have a valid UK Driving Licence.

Job responsibilities

- To work across the services which provide Urgent Community Response, Discharge to Assess and short term rehabilitation.
- To provide care to people in their own homes to prevent admission/ readmission to hospital by providing rapid assessment of patients in the community over seven days.
- Undertake comprehensive holistic assessment of patients in both planned and unplanned situations, be able to prioritise clinical need, initiating investigations, determine a differential diagnosis and implement a plan of care reflecting evidence based guidance.
- To sustain agreed standards of care in to order to maintain integrity and dignity of all patients in an environment which is conducive to delivering care in their usual place of residence or temporary place of residence including bed based rehabilitation.
- Undertake triage within the service determining clinical priority of calls, working as an coordinator ensure teams are dispatched to respond to the referrals.
- Work collaboratively with multi professional teams to ensure practice is efficient, effective and evidence based.
- To develop his /her scope of professional practice in order to support the team to meet service requirements in accordance with professional guidelines.
- To ensure the provision of expert knowledge to all users of the service.
