Job summary

An exciting opportunity has arisen for a dynamic, forward thinking and enthusiastic registered Specialist Practice Nurse to join our multi-disciplinary team here at Glenroyd Medical on a part-time basis.

We are looking for an individual to support our current Clinical Management Team in undertaking our chronic disease management services, specialising in diabetes management, within primary care, as well as undertaking reviews of clinical policies and procedures as and when required.

The successful candidate will work as part of a multidisciplinary team in a patient facing role, across our two sites.

The post-holder will play a pivotal role in providing advanced nursing care to patients with long term chronic diseases, ensuring they receive comprehensive support, education and management tailored to their individual needs.

The role will be focused on planned reviews and completion of personalised care and support plans, accessing team members such as Social Prescribers and Clinical Pharmacists for expertise where needed.

The role will be part time (25hrs per week) and salary will be in line with NHS Band 7, dependent on experience and existing qualifications.

Main duties of the job

The Specialist Practice Nurse will be competent and confident in long term chronic disease management, specialising in the treatment of diabetes management, including insulin initiation. They will take, not only a lead role, but a proactive role, diagnosing, managing, and effectively treatment patients with long term conditions, ultimately, creating a more streamlined journey for the patient overall.

We are looking for an Independent non-medical prescriber who is compliant in both issuing and reviewing medications to patients with regards to their effectiveness and needs of the patient, as well as working in line with local and national guidance, in addition to our own practice policies.

If a non-medical prescribing qualification is not held, the willingness to complete this upon commencement of starting the role is essential.

The role itself is pivotal to improving quality of care and operational efficiencies so requires motivation and passion to deliver excellent service with general practice.

Glenroyd Medical is set across two sites, currently serving a population of approximately 15,000 patients.

We currently have an innovative and extensive team of highly skilled clinicians, including GP's, Advanced Clinical Practitioners, a designated Visiting Team, as well as Clinical Pharmacists, Practice Nurses and HCA's. We are also a Training Practice meaning we continue to support Doctors who are mid-way through their medical training.

Our Practice have a very knowledgeable and experienced administration team, including our Secretaries, Admin and Reception team who support our team of clinicians as well as our patients to ensure the smooth running of the Practice.

Alongside North Shore Surgery, we also form Blackpool North Primary Care Network, which allows us to provide additional clinical and admin support to further enhance the practice team.

Job responsibilities

Job Summary:

The post holder will play a pivotal role in providing advanced nursing care to patients with long term chronic diseases, ensuring they receive comprehensive support, education and management tailored to their individual needs as well as the provision of preventative health care to the practice population.

Job responsibilities:

  • Deliver specialist diabetes care, assessment and treatment to our diabetic patients, including initiation of insulin therapy.
  • Work with other health care professionals to diagnose, manage and monitor long term conditions, including non-drug based treatment methods using a management plan and in line with National and Local policies and practice needs.
  • Assess, diagnose,plan implement and evaluate treatment/intervention that promotes health and wellbeing, as well as care for patients presenting with an undifferentiated diagnosis.
  • Clinically examine and assess patient need from a physiological and psychological perspective and plan clinical care accordingly.
  • Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
  • Prescribe, review and instigate medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, NICE guidance and practice protocols.
  • Work with patients in order to support compliance and adherence to prescribed treatments.
  • Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self care.
  • Advise, support and where appropriate administer vaccines for patients travelling abroad.
  • Implement and participate in vaccination and immunisation programmes for both adults and children.
  • Able to recognise and manage anaphylaxis according to current UK guidelines.

Duties will vary from time to time under the direction of the Partners/Practice Managers dependent on current and evolving practice workload and staffing levels.

The successful candidate will also be required to perform an enhanced Disclosure Barring System (DBS) check.

  • Substantial post registration experience in Primary Care
  • Experience of using clinical software: Doman, Emis Web
  • Evidence of continued professional development
