Job summary
This is a Team Leader Support Worker opportunity with Libertatem Healthcare Group, a provider of care and support services for clients with personal injuries or medical negligence. The role is based in Plymouth and involves supporting a client with a brain injury, ensuring their safety and independence, and building a strong relationship with them.
Main duties of the job
As a Team Leader Support Worker, you will be responsible for supporting our client in the community, ensuring their safety at home, and assisting them to live as independently as possible. You will work closely with the team and case manager, building a strong relationship with the client. Your duties will include completing rotas, mileage forms, and attending multidisciplinary team meetings.

Are you passionate about making a positive impact in someone's life? We're looking for a dedicated Team Leader Support Worker to assist one of our client in Plymouth. You'll play an important role in helping him with day-to-day activities, building his independence, offering companionship, and driving him to his appointments. Our client enjoys going to the gym and playing football.What You'll Be Doing:
- Supporting our client in the community
- Ensuring our client feels safe at home
- Assisting our client to live as independently
- Working closely with the team and Case manager
- Building a strong relationship with our client
- Ensuring smooth running of the package
- Completing rotas
- Mileage forms
- Attending Multidisciplinary Team meetings
What We're Looking For:
- Experience in supporting individuals with brain injuries
- A full UK driving licence and access to your own vehicle
- Previous leadership experience in care settings
- A commitment to delivering compassionate, high-quality care
- Strong communication and interpersonal skills
- Ability to work collaboratively as part of a multidisciplinary team

Person Specification
Qualifications
Essential
- You should have experience in supporting individuals with brain injuries, a full UK driving license, and access to your own vehicle. Previous leadership experience in care settings and a commitment to delivering compassionate, high-quality care are also required.
