Job summary

We are looking for an enthusiastic candidate with good communication skills and experience of team working. The candidate will be able to demonstrate an ability to motivate and reassure patients as well as providing rehabilitation, as recommended by the physiotherapist.

The post holder will work as part of the Optimise Pain Rehabilitation Unit, in a multidisciplinary team, to assist with group rehabilitation programmes for patients with long term pain. The applicant will be provided with training and support to help develop the necessary skills for working with this population of patients.

The role also involves working with small groups of patients in the gym independently and in hydrotherapy.

Main duties of the job

To work independently and autonomously, providing assessment, support, care, treatment and advice to individuals, within guidelines and protocols identified for the role.

To work within a multi-professional team undertaking practitioner role.

To plan and implement treatment and care to patients within professional guidelines e.g. delivery of rehabilitation programmes etc.

To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.

To take responsibility for the care and support of delegated patients.

Deliver treatment/care as specified in a rehabilitation programme outline, under the guidance of a registered practitioner.

To be responsible for collating patient outcome data & entering on data base.

Main Purpose of Job

Section 2 - Job Summary

Main Purpose of Post

To form part of a multidisciplinary pain rehabilitation team undertaking group rehabilitation of patients referred to the service.

The MDT includes medical staff, clinical psychology, employment advisors and physiotherapists.

To visit patients at home, and conduct rehabilitation according to a research protocol.

Main Duties and Responsibilities

Clinical

  • Responsibility for the day to day care of patients including assessment and monitoring.
  • Delivery of components of rehabilitation programmes under the authorisation of the qualified practitioner, both in the physiotherapy department and within patients homes.
  • To tailor treatments to individual needs of each patient, applying knowledge learnt through formal learning and experiential learning.
  • To lead relaxation classes.
  • To conduct follow up assessments to evaluate effectiveness of treatment outcome & provide ongoing support to patients.
  • To lead groups in hydrotherapy, including self management group.
  • To ensure the needs and wishes of the individual are documented.
  • To obtain consent for any treatment undertaken.
  • To enable patients to reach their optimum level of independence.

Communication and Key Relationships

The post holder will interact with all members of physiotherapy, medical and other allied professionals related to in patients.

  • To communicate with multi disciplinary team members, patients and carers.
  • To record activity onto CRS.
  • Communicate with acute Trusts, GPs, and other support services
  • To communicate any concerns about vulnerable adults, suicide, child protection to most senior clinician present immediately.

Management

  • Organise and manage own time according to delegated workload to achieve the patients goals.

Quality/Clinical Governance/Research and Audit

  • To maintain accurate and cotemporaneous clinical records of care given.
  • Collect PROMS data
  • Enter data onto database & prepare summary reports
  • The post holder is required to familiarise him/herself with and comply with the Trusts policies and procedures.

Professional Development and Education Responsibilities

  • To support junior members of staff and students, promoting and monitoring best practice.
  • Develop and maintain skills required for patients within a rehabilitation setting in line with appraisal and KSF.
  • To participate in supervision and appraisal and take responsibility for own professional development.
  • Undertake statutory and mandatory and other training as requested.

Budget

  • To be aware of budgetary constraints and support lean productive tools
  • To maintain equipment & undertake annual stock take / asset verification
  • To participate in ordering and monitoring of stores, equipment and other appropriate tasks.

Policies and Procedures

  • To follow departmental policies and procedures relating to rehabilitation classes
  • To be aware of, keep up to date with and ensure that self and staff adhere to the following at all times:

Section 6 Working Conditions/Efforts

  • Physical Effort:- There is the expectation that the role will require moderate physical effort for short periods.
  • Mental Effort:- Typing / data inputting skills, following care plans to provide appropriate treatment.
  • Emotional Effort:- there will be occasional exposure to distressing or emotional circumstances.
  • Working Conditions:- There is an expectation for the post holder to occasionally come into contact with highly unpleasant conditions e.g.: odours, body fluids.
