As a key member of the Lifelong Services reviewing team, you will contribute to improving the wellbeing and independence of individuals with physical disabilities, learning disabilities, and autism.

What the Job Involves

This role involves working as an Occupational Therapist within the Countywide review team, focusing on adults 18 and upwards with various disabilities and health conditions. You will conduct strengths-based assessments, develop and implement personalized support plans, and use evidence-based interventions to achieve defined outcomes. Additionally, you will perform mental capacity act assessments and contribute to Continuing Health Care checklists and decision support tools. Your work will include recommending assistive technologies, home adaptations, and collaborating with other professionals in a dynamic reviewing service.
