North East Derbyshire (North) Adult Social Work Team are looking to appoint enthusiastic and motivated Social Care Practitioner's (SCPs). There are 2.5 full and part-time, temporary vacancies available, where this is a good opportunity for new and existing SCPs wanting to move into community-based adult social care. The team are highly supportive and work on an agile, flexible basis between the office, at home and in the community depending on the needs of the service. You will have the opportunity to work alongside other Social Care Practitioners and Social Workers, including link working with Health and other partner agencies.

The successful applicant will be able to demonstrate the necessary skills, knowledge and experience to undertake this role in order to provide a safe, accountable, cost effective and efficient service to our clients which helps them to achieve their outcomes. Key responsibilities will include carrying out assessments and reviews for individuals with less complex needs as well as arranging the provision of services to meet Care Act (2014) eligibility. It is expected that you will be committed to working in a person centred and strengths-based way with clients, carers and their families to maintain and maximise levels of independence. This may include interventions across home visits, residential care and nursing settings.

The team have a dedicated duty, though there may be some cover of incoming, duty work which will involve further triage of new referrals to the team and/or support for adults and carers who require a more urgent response.
