The service you will work in....

Hammersmith and Fulham Housing Support and Inclusion Service is the largest generic floating support service within the borough. Support and assistance are provided to vulnerable adults living independently within the community. You will be managing and supporting housing floating support workers to deliver a floating support/outreach service, with a primary focus on housing and benefit-related support for vulnerable adults aged 16 and over who are referred to the service.

A day in this role looks like....

As a Floating Support Case Workerat Hestia, you will play a crucial role in our Housing SupportInclusive Service. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.

A day will never look the same and some of the things you can expect to do/be responsible for are:

  • Empower a caseload of clients with a range of complex health and support needs and their carers to manage their health and wellbeing, maintain their independence, identify their strengths and achieve their goals and aspirations, working in partnership with our subcontractors as required in order to achieve this, under the direction of the Team Leader.​​
  • To clearly articulate the aims and objectives, pathway and service model to people using the service so they can fully participate in and co-produce their support.
  • ​To attend weekly drop-in surgeries and wellbeing and recovery sessions as part of the contract and to participate in facilitating such sessions on occasions​​.
  • ​Empower service users to engage effectively in their liaisons with professionals within the community such as social work teams, housing providers, health professionals, JCP etc.
  • ​Work flexibly as part of a team to ensure service coverage and that contractual obligations are met.​​
  • ​Develop co-produced group activities, peer support networks and other community-based approaches to empowering service users to achieve their goals and aspirations and to encourage service users to access these opportunities. ​​
  • ​Actively monitor the health, wellbeing, safety and security of the service and its users ensuring all concerns are followed up promptly and in line with procedures. ​​
  • ​Participate in the core work of the service, providing support to colleagues, carrying out tasks allocated by the service manager and using initiative to identify and deal with additional tasks as they arise​​.
  • ​Ensure that administration is done in good time; that detailed, concise and accurate records are created contemporaneously, and that confidentiality is appropriately maintained​​.
  • ​Carry out duties in line with organisational policies and procedure​​.

  • Ability to deliver high quality floating, outreach or accommodation-based support services to people with a range of needs and risks​​​.
  • Good knowledge of welfare benefits and housing options and legislation related to statutory duty​​​.
  • Knowledge of health and safety relevant to role as a Floating Support Worker and working in Service Users' homes​​​.
  • Ability to work with people with a range of different support needs, aspirations for their future and associated risks​​​.
  • An ability to undertake client-focused key working, support planning and risk assessments to maximise Service Users' independence around a range of areas​​​.
  • Good understanding of the range of issues, diagnoses and treatment relevant to the client group including mental health and substance misuse. ​​​
  • Commitment to equal opportunities and to promoting service users' rights, privacy, dignity, choice and control​​​.
  • Ability to liaise with external professionals to inform and develop Service Users' support package​​​.
  • Willingness to actively engage in undertaking relevant personal development and training​​​.
  • Ability to travel across Hammersmith and Fulham to client's homes providing key work and attend weekly drop-in surgeries and wellbeing sessions organised and led by the service​​​.
  • Previous experience of using computer-based systems to accurately record work and client details​​​.
