The service you will work in....

Talgarth Road Care Home is a registered residential care home regulated by the Care Quality Commission (CQC). The property is located in Hammersmith and Fulham with good access to public transport. Support is provided to 10 men / women with severe and enduring mental health needs who require support 24 hours a day.

The main purpose of this role is to provide high quality care and support to service users bringing the care/support plans to life and carrying out the actions agreed.

You will work in a service that is warm, vibrant, caring and innovative, with the service users at the heart of everything we do. You will be a part of a competent team, dedicated to giving our service users quality care and support.

A day in this role looks like....

As a Support Workerat Hestia, you will play a crucial role in our Mental Health and Complex Needsservice. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.

A day will never look the same and some of the things you can expect to do/be responsible for are:

  • Manage a caseload of Service Users with a range of complex health and support needs and their carers to manage their health and wellbeing, maintain their independence, identify their strengths and achieve their goals and aspirations, working in partnership with our subcontractors as required in order to achieve this, under the direction of the Team Manager.​
  • To provide day to day care and support of the service users in line with the actions identified in the care/support plans. ​
  • To assist them to attend educational, leisure and health related activities, accompanying them as appropriate. ​
  • To assist with planning, review, and implementation of support / care plans with the participation of service users. ​
  • To assist with the administration of medication, supporting service users to transition towards managing their own medication where appropriate. ​
  • Actively monitor the health, wellbeing, safety and security of the service and its users ensuring all concerns are reported, followed up promptly in line with Hestia procedures.​
  • To carry out domestic tasks (including cooking and cleaning of the home) when appropriate. ​
  • To liaise effectively with other agencies involved in the care/support of service users.​

We are looking for candidates who bring a combination of passion, expertise, and dedication to the table. To excel in this role, you should possess:

  • Good Knowledge of issues related to signs, symptoms and treatments of mental illness. Ability to assess service users' declining mental health and take appropriate action. ​
  • Basic awareness of health and safety issues.​
  • Willingness to cook and clean with the service users and to assist with personal care.​
  • Willingness to work unsociable hours within a rota system including weekends, evenings and sleep-ins.​
  • Willingness to maximise independence by motivating and supporting service users towards their goals. ​
  • Willingness to travel and work across Hestia services when required. ​
  • Promotes a service user-oriented organisation by ensuring that all activity is aligned to identify service user needs. ​
  • Good Literacy and numeracy skills. Working knowledge of basic functions in MS Word and Outlook. Ability to use an I.T. based Support Planning, Risk Assessment and Keyworking system​.
  • Knowledge and understanding of safeguarding issues and ability to address them appropriately. ​
