As we continue to grow, we are looking for Nurse Care Practitioners for Continuing Healthcare (CHC) reviews and assessments who want to be part of a vibrant team who are making a difference.

Main responsibilities:

Working within the National Framework for NHS Continuing Healthcare (CHC) and/or the Care Act 2014, you will chair Multi-Disciplinary Meetings (MDTs), and complete reviews and assessments on behalf of Integrated Care Boards (ICB) and Health Boards. These will be done with the individual, the provider and their family (or their representatives), and are held virtually on MS Teams or Zoom.

  • Able to communicate very sensitive, complex condition-related information to individuals and/or relatives where there may be barriers to understanding, requiring persuasive and influencing skills, and demonstrating empathy and reassurance.
  • Highly skilled in engaging others in collaborative working through effective negotiation, persuasion, and interpersonal skills.
  • Being able to deal with highly sensitive, highly complex information in relation to changes to service provision.
  • Excellent written communication skills with the ability to produce reports that can be understood by both individuals, their representatives and clinical audiences, often in relation to complex or contentious subject matter.
  • Analytical and judgement skills for assessing and interpreting specialist conditions.
  • Excellent planning and organisational skills.
  • Self-motivated and experience of achieving KPI's.
  • Able to liaise with other services and professionals, both internal and external.
  • Experience of planning provision of care for individuals.

Experience and qualifications required

Essential:

  • Registered Nurse
  • Minimum 12 months post qualification experience
  • Previous experience working within an NHS Continuing Health Team or in the community completing or inputting to CHC assessments and reviews
  • Ability to write clear and comprehensive reviews and assessments to a high standard
  • Able to complete person-centred care and support plans
  • Computer literate, specifically for Microsoft suite including Outlook & Excel
  • Experience in similar work; reviewing, evaluating, and planning adult health and social care.
  • Good working knowledge of CHC Framework 2022
  • Knowledge of Care Act 2014 and Mental Capacity Act 2005
