Who You Are
- Educated to degree level with a professional qualification relevant to the role (e.g., CSS/CQSW, DipSw, Dip COT, or Degree in Occupational Therapy).
- Registered with HCPC.
- Knowledge of the Care Act 2014, the Mental Capacity Act 2005, and other relevant legislation.
- Demonstrable understanding of principles and application of excellent adult social care.
- Strong literacy, numeracy, interpersonal, and IT skills.
- Experience in a similar role in a Local Authority and using case-based ICT systems preferred.
- Demonstrated commitment to personal and team development, equality and diversity, and continuous service improvement.
- Ability to communicate effectively across all levels and build effective working relationships.
- A minimum of 1 year's experience as a qualified practitioner is desirable.
- Experience in commissioning and coordinating service delivery is a plus.
What the Job Involves
- Managing a service caseload including customers with complex needs.
- Accepting referrals, undertaking screening, and carrying out assessments of needs for customers.
- Applying eligibility criteria as identified in the Care Act and adhering to its requirements.
- Providing professional supervision to non-qualified staff.
- Maintaining professional qualifications, memberships, and registrations required for the role.
- Developing and maintaining effective working relationships with internal and external partners.
- Ensuring staff and public safety, dealing effectively with escalations, and preventing/diffusing difficult situations.
- Keeping up-to-date with developments, products, and information related to the service and the council.
- Ensuring proportionate risk assessments for both customers and carers.
- Undertaking reviews and reassessments of customers with long-term needs as required.
- Providing a proportionate assessment of needs for customers and developing outcome-based support plans.
- Ensuring appropriate levels of long-term support for customers to live as independently as possible.
- Undertaking functional, moving and handling assessments, recommending and commissioning equipment/structural adaptations for customers.
- Planning and implementing individual goal-setting plans with customers.
- Demonstrating a commitment to professional development and coaching of other team members.
- Supporting team leaders and service managers in developing short and long-term plans for the team.
