Job Description
Who You Are
- A qualified Social Worker with a recognised degree in Social Work, DipSW or CQSW, and Social Work England Registration.
- Experienced in carrying out comprehensive social work assessments using the Care Act wellbeing principles.
- Proficient in providing information, advice, and signposting to enable service users to make informed decisions.
- Able to work collaboratively with service users, carers, and a wide range of stakeholders across various organisations.
- An effective communicator, both in writing and verbally, capable of presenting complex information sensitively and appropriately.
- Organised and able to manage your own workload while coordinating with team members.
- Committed to continuous professional development and supporting equalities within the workplace and community.
- Knowledgeable about the Care Act, Mental Capacity Act/DOLS, and London Multi-Agency Adult Safeguarding policy and procedures.
- Technically skilled with a good working knowledge of IT tools including databases, word processing, and spreadsheets.
What the Job Involves
- Participating in assessments, care planning, and safeguarding under the supervision of an experienced practitioner.
- Completing assessments and support plans in accordance with the wellbeing principles of the Care Act using a strengths-based approach.
- Managing a caseload, conducting robust assessments in partnership with service users, their carers, and multidisciplinary partners.
- Identifying outcomes that need to be achieved and helping people meet their long-term needs and aspirations through asset-based goals.
- Providing a high-quality and effective social work service to respond to the needs of adults, carers, and their families.
- Assessing needs, planning, and delivering focused interventions to safeguard adults and promote positive outcomes.
- Maintaining accurate and timely documentation and records of all work undertaken.
- Proactively promoting activities and services that improve health and wellbeing and help people live independently.
- Collaborating with other health, mental health, and social care services to ensure smooth transitions and support provision.
- Applying the principles of personalisation and using person-centered approaches to facilitate service user involvement and choice.
- Leading and coordinating safeguarding adult cases and presenting information in case conferences and meetings as necessary.
- Undertaking training and development, including in the area of safeguarding, and staying up-to-date with legislative frameworks and practice developments in adult social services.
