Job Description

Who You Are

  • Qualified social worker with a SW degree or equivalent.
  • Current registration with HCPC.
  • Minimum 2 years post-qualification experience in statutory Adult's Social Care service.
  • Possess skills and knowledge of NHS and Community Care Act, Mental Health Act, and other appropriate legislative frameworks.
  • Proficient in assessment frameworks, human growth and development, and professional decision making.
  • Capable of critical reflection and analysis in complex cases.
  • Excellent communication, organizational, and partnership working skills.
  • Committed to professional development and adherence to ethical standards.
  • Ability to undergo satisfactory Enhanced Disclosure and Barring Service (DBS) check and meet special requirements including being a casual car user.
  • Must be legally entitled to work in the UK and adhere to the Council's no smoking policy.

What the Job Involves

  • Managing a complex and varied caseload with appropriate supervision, reflecting team functions and service policy.
  • Working within legislative frameworks, including court representations.
  • Making complex decisions involving professional judgment, service user involvement, and needs-led assessments.
  • Identifying and assessing levels of risk and need, conducting safeguarding investigations, and providing protective/supportive services.
  • Preparing needs/care and support plans, and commissioning personalized packages of care in consultation with other agencies.
  • Negotiating personalized care arrangements considering available resources and budgets.
  • Conducting direct work with adults and their families in alignment with care plans.
  • Collaborating with internal and external colleagues across multi-agency boundaries.
  • Handling specific responsibilities and out-of-hours duties as required.
  • Communicating effectively in high-risk situations and developing others' communication skills.
  • Representing the department in multi-disciplinary forums and occasionally chairing meetings.
  • Modeling critical reflection, evidence-based decision-making, and fostering inclusive practice.
  • Engaging in professional supervision and development reviews to enhance practice and career growth.
  • Overseeing the management, teaching, and assessment of social work students on placements.
  • Ensuring high-quality, timely reports and proper authorization of case expenditures.
  • Maintaining accurate, up-to-date data entry and safeguarding client information in accordance with Data Protection Act 1998.
  • Performing additional duties as directed to meet service demands and contributing to the overall service and council development.
  • Complying with Health and Safety and Equality and Diversity policies.
