Job Description

Who You Are

  • Qualified and experienced social worker with a degree in social work or equivalent.
  • Current registration with HCPCC.
  • Minimum of 2 years post-qualification experience in statutory adult's social care service.
  • Knowledgeable in NHS and Community Care Act, Mental Health Act, and other relevant legislation.
  • Skilled in applying needs-led assessment and planning with a strong understanding of human growth and development.
  • Proficient in critical reflection, analysis, and making person-centred judgments.
  • Effective communicator, both in written and verbal forms, able to produce high-quality reports and plans.
  • Adept in partnership working, especially with individuals and their families, and demonstrating client empathy.
  • Organized and capable of managing fluctuating caseloads and meeting deadlines.
  • Committed to professional development and adherence to ethical standards including the HCPC Code.
  • Possesses a valid DBS check and legally entitled to work in the UK.

What the Job Involves

  • Managing a complex caseload with appropriate supervision, guidance, and support.
  • Undertaking work within legislative frameworks and preparing written statements for court hearings.
  • Making complex decisions related to cases ensuring professional judgment and service user involvement.
  • Identifying and assessing levels of risk and need, undertaking safeguarding investigations.
  • Preparing needs and care support plans in consultation with other agencies leading to personalized care packages.
  • Negotiating personalized care arrangements within budget constraints.
  • Conducting direct work with adults and their families aligned with care plans.
  • Collaborating with both internal and external colleagues across multi-agency boundaries.
  • Undertaking specific responsibilities such as the role of AMPH and potentially engaging in out-of-hours duty.
  • Representing the department in multi-disciplinary forums and potentially chairing meetings.
  • Ensuring high-quality, up-to-date data entry and accurate records management.
  • Overseeing the management, teaching, and assessment of social work students on placements.
  • Monitoring and promoting health, safety, wellbeing, and emotional resilience.
  • Contributing to team development and attending team meetings.
  • Ensuring client information data is lawfully gathered and divulged in accordance with the Data Protection Act 1998.
  • Potentially contributing to the Out of Hours Service and promoting equality and diversity in all aspects of work.
  • Position based primarily in the South of the Borough with potential for hybrid working between Bootle office and home.
