Job responsibilities

KNOWLEDGE AND EXPERIENCE

Learning Disability and Autism Programme Senior Practitioner (inpatients and admission avoidance).Make a difference to some of societys most vulnerable people as a Senior Practitioner in the Learning Disability and Autism Programme Team.

  • Make a difference help ensure people who have a diagnosis of Autism and/ or Learning Disability who are admitted to hospital receive safe and effective care with a focus on discharge planning from day one of admission.
  • How? You will be part of a small, dedicated team focussed on this niche work.

  • You will also be linking in with the team leads who are working to avoid inappropriate hospital admissions, for those who have a diagnosis of Autism and/or Learning Disability
  • We are responsible for the admission avoidance and inpatient aspects of the Learning Disability and Autism Programme across Hampshire, Southampton and The Isle of Wight. The team now covers all age continuing care and continues to expand to reflect the importance of this work and an experienced and skilled senior practitioner is required.

What youll be doing:

  • Completing work in a manner that ensures compliance with local and national related policy.
  • Supporting discharges from Acute hospitals, Assessment and Treatment units and any other type of inpatient unit which provides care for people with learning disability, Autism or both.
  • Completion of Care and Treatment Reviews (CTRs), to support discharge from hospital settings.
  • Completion of face to face, regular, commissioner oversight checks at 6/8 week intervals for inpatients in hospital settings, to ensure patients are safe and receiving effective care and discharge planning.
  • The post holder will work closely with hospitals, community services and care providers to develop systems and processes to facilitate a smooth processes and responses to a range of stakeholders.
  • Commissioning care packages which are in accordance with locally agreed protocols
  • Setting up of Personal Health Budgets

Person Specification

Knowledge, Training and Experience

Essential

  • Registered Health or Social Care Professional with current registration with professional body i.e. NMC, HCPC or SWE. including AMHP's and mental health social workers or significant relevant experience (LD/MH/LDAP teams only).
  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in health or social care.
  • Experience of assessment of health and social care needs - care planning and coordination.
  • Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues (LD/MH/LDAP team roles).
  • Knowledge of complex neurological conditions and experience of working with clients with complex brain injuries. Specific knowledge of the National Service Frameworks and appropriate national policy related to rehabilitation of people with acquired brain injury/ neurological dysfunction (Specialist Rehabilitation / ABI team).
  • Ability to work effectively under stressful conditions in a rapidly changing environment.
  • Good knowledge of Mental Capacity Act and relevant legislation (for DOLS team)
  • Experience of implementing and managing Personal Health Budgets.

Desirable

  • Demonstrable experience of care/case management.
  • Knowledge of and experience of NHS Funded Continuing Health Care criteria - guidance and policies and other legal frameworks as required (Children and Complex Care, s117 Aftercare, Mental Health Act).
  • Experience of developing care packages.
  • Experience in communications and stakeholder management.
  • Effective working knowledge of Microsoft software.
  • A good understanding of the health and social care environment and roles and responsibilities within it.

Communication and Relationship Skills

Essential

  • Clear communicator with excellent writing, report writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences.
  • Skills for communication on complex matters and difficult situations, requiring persuasion and influence.
  • Skills for nurturing key relationships and maintaining networks.
