11) Ensure effective Safeguarding and Risk Management Act as the lead manager in safeguarding concerns / enquiries including assessment and management of risk, knowing how to intervene proportionately and ensuring people are protected from harm, while protecting their human rights. Provides professional leadership in situations which are complex or ambiguous. Leads on partnership working with other organisations, community groups and community members in a variety of settings to achieve positive outcomes for service users and carers. Identifies and ensures the team actively engages with, assists and provides advice to carers to enable them to sustain their caring role. Provides expert knowledge of safeguarding processes, policy and case law Works within Hertfordshire safeguarding frameworks to ensure vulnerable adults are protected. Undertakes relevant audits as well as quality monitoring of social work Accurately recording and keeping records up to date. 22) Addresses adversity and social exclusion Acts as the lead professional when an adult is at risk of social exclusion and assist people to deal with adverse circumstance such as poor health, poverty, inadequate living conditions; as well as maximising the strength of individuals, their families and their communities. Actively promote Equality and Diversity and challenges discrimination 33) Promote independence and autonomy Acts as the lead professional where a person is severely constrained by social or family circumstance and provides support to achieve a reasonable degree to independence and autonomy. Uses a person centred approach, developing creative and personalised solutions to assist people to manage their lives independently for as long as possible. Promotes independence, well-being: choice and control within a personalisation framework and flexible use of self-directed support. Work with service users and carers across the county. Meetings and appointments will be across several locations and in peoples homes. Undertakes preventative work to promote independence both with individuals and within wider project work 44) Prevention and Early Intervention Acts as the lead professional when a persons health or capacity is deteriorating or likely to deteriorate (without intervention) and the alternative may be premature admission to institutional forms of care or a legal intervention by the state 55) Demonstrate HPFT Values and Professional Standards Carries out duties in a timely and responsive manner, in line with Health and Care Professions Council (HCPC), Social Work England standards, Professional Standards, Codes of Conduct. Maximises efficiencies in care planning through consideration of charging systems and other funding streams. Keeps and maintains accurate, up to date service user records, in line with professional requirements and departmental recording methods. To identify safeguarding plans to mitigate risk to the person; oversee gathering of relevant information; identify scope and terms of reference of any enquiry and decision making tool; apply safeguarding eligibility criteria; record and keep records up to date; hold and chair strategy discussions or meetings; chair case conferences, ensuring outcomes are recorded; and identify need for other assessments or interventions required. Leadership and Staff Management Responsibility Mentors and supervises staff, delivers induction programmes and training. Supports workforce development initiatives and provides expert advice regarding the social work workforce. Actively contributes to practice development and leads local peer group meetings. Has an in-depth knowledge on policy, procedure and practice and disseminate this to other team members using appropriate methods. Takes responsibility for own professional development in line with own Personal Development Plan and performs at an appropriate level of competence. Takes part in specific departmental projects and undertake tasks as required by ACMHS Service Manager. To maintain ones own Professional Registration and development. To actively participate in and seek clinical and professional supervision and appraisal in line with organisational policy. Managers have a duty to promote and support staffs personal health and wellbeing at all times, this includes both physical and mental health and wellbeing. Managers have a responsibility to support staff by directing employees to services that are available to everyone to help in managing health and wellbeing. They should be open and approachable as well as proactive in discussing and agreeing a process to monitor an employees mental and physical health. Financial Responsibility Maximises efficiencies in care planning through consideration of charging systems and other funding streams. Service Development and Improvement Describe the responsibilities of the job for development and implementation of policy and/or services, e.g., write, implement or comment on a policy. The Adult Safeguarding Advanced Practitioner will work with management to develop practice within the service. Lead on peer development and learning in conjunction with development programmes and workforce strategy and provides coaching, mentoring and supervision as appropriate. Support workforce development initiatives and provide expert advice regarding the social work workforce. To participate in specific departmental projects as necessary Communications Exceptional interpersonal and written communication skills with individuals, Service Users, Carers, Staff and Colleagues, Other Professionals and members of the general Community and Community Groups. To convey the required information accurately and concisely while maintaining the appropriate levels of confidentiality at all times. Communication will be Face-to-Face, Written Document or referral to IT System Records as applicable.
