Clinical Role Function as a key member of the clinical team at St Marys, delivering primary care services to patients. Deliver a high standard of patient care as an Advanced Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base. To deliver clinical assessments and primary care services for patients of St Marys surgery, both remotely (over telephone) and face-to-face, including in the surgery and in the patients own home. Provide scheduled (booked) care and on-day assessment for patients presenting to the practice with more urgent needs. To undertake assessments as required in patients own home and other community settings as appropriate. Make professionally autonomous decisions for which the post holder is accountable. Manage an undifferentiated caseload of patients presenting with acute and chronic, complex health needs. Contribute to the management of care for patients within a list based primary care model, contributing to an MDT/Clinical firm to promote continuity of care. Manage their own clinical workload in general practice responding effectively to patient need and ensuring patient choice and ease of access to services. Provide assessments and care for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan. Maintain accurate and contemporaneous records, utilising computer systems where appropriate and consider the Caldicott Principles in relation to all data handling. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports. Provide safe, evidence-based, cost-effective, individualised patient care. Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care. Prescribe safe, effective, and appropriate medication as defined by current legislative framework and local prescribing standards. Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills. Refer patients directly to other services/agencies as appropriate including but not limited to services of the Primary Care Network, acute hospital services, community services, mental health services and local social services. Support the other clinicians within the practice in developing and delivering specialist primary care services as required. Work within the practice guidelines, policies, and protocols. Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery. Provide ongoing care to patients with more complex needs, including liaising with appropriate multi-disciplinary team to create appropriate care plans and review them as required. Professional role The post-holder will Promote evidence-based practice using the latest research-based guidelines and the development of practice-based research. Ensure practice which is of high quality, that meets regulatory and contractual requirements and to contribute to the development of clinical governance arrangements across St Marys services. Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review. Maintain their professional registration with the relevant governing body for their profession. Remain clinically competent in relation to their job-plan. Participate with/ receive clinical supervision from the Partner (Advanced Practitioner Lead) on a regularised and ad-hoc basis. Contribute to peer-to-peer and other supervision arrangements within St Marys and the Sea City PCN Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained. Work within the latest Code of Professional Conduct relevant to profession. To undertake continuous professional development and to work with the Advanced Practitioner team and wider team to maximise the safe scope of advanced practice within the St Marys clinical team. Record accurate consultation data in patients records in accordance with the latest relevant guidance and other pertinent standards. Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation. Work collaboratively with colleagues within and external to the practice. Pro-actively promote the role of the Advanced Practitioner within the Practice, and externally to key stakeholders and agencies. Team role The post holder will Demonstrate and model positive behaviours that are aligned with the values of St Marys surgery (refer to the Employee Handbook). To support and encourage these behaviours across the Advanced Practitioner and wider St Marys team. As a clinician be available to support other healthcare team members and to accept referrals from other team members. Liaise with all members of the Primary Health Care Team and other agencies local authority, social services, secondary care, voluntary sector, and Primary Care Trust to assure appropriate and timely care is provided for the practice population. Participate as a key member of the multi-professional St Marys team through the development of collaborative and innovative practice. To work within the Advanced Practitioner team and with wider clinical team and senior management team to develop and implement strategies which promote excellent patient care and evidence-based practice. Contribute to the development of the Advanced Practitioner team and wider practice multi-disciplinary team as a high functioning team where team members are supported to share responsibilities and lead on agreed areas of work. To work with peers to foster strong levels of autonomy within the team. To promote an environment where issues and challenges are addressed collectively. Value all team members across St Marys surgery, the Primary Care network and associated partner health and social care agencies. Work with Advanced Practitioner colleagues and the wider St Marys clinical team ensure that services are patient centred. To ensure that patients are actively engaged in the planning and delivery of their care. To promote the voice of patients and carers and to ensure that feedback, complaints, and other representations from patients are thoroughly considered in the planning and delivery of services. To ensure that all concerns and complaints raised by patients are managed in accordance with the Practices complaints procedures. Delegate appropriate tasks to other members of the primary care team whilst still maintaining responsibility for this area. Be a contribute to the clinical and operational leadership of the St Marys team, supporting the practice in achieving its strategic and business plans within the available resources. Develop networks and opportunities for collaborative working with Advanced Practitioners across the Sea City Primary Care network, with community and mental health services and other relevant stakeholders. Attend meetings as required. Ensure that orders for supplies are within a reasonable budget and one is constantly seeking to reduce costs to the Partnership. Complete records, audits, reports and respond to appropriate questions and requests as required to support the continued improvement of clinical services at St Marys. Support and contribute to effective communication channels across the St Marys team. Work within and contribute to the practices clinical governance framework and relevant policies including but not limited to the investigation of significant events, sharing of best practice, and learning and infection prevention and control. Actively participate in the delivery of QOF targets and other St Marys and Sea City PCN contractual arrangements To contribute to relevant processes and activities which promote compliance with regulatory and contractual requirements. Ensure the appropriate delivery of such processes and that effective records are appropriately filed to provide evidence of compliance. Provide assurance reports as required on a regular and ad-hoc basis to the Partners and Senior Management team. To be a senior member of the St Marys Clinical team. To contribute to the overall day-to-day operational management of St Marys Services. To be an advocate for Advanced Practitioners. Represent the interests and viewpoint of Advanced Practitioners both within the practice and with partner agencies. To contribute to the exploration, planning, delivery, and evaluation of service developments within St Marys Surgery. To support the effective delivery of projects to improve patient care, the sustainability services, staff-wellbeing, and the role of Advanced Practitioners within Primary Care. Contribute to the delivery of specifically identified services, projects and audits as required. To support the placement and supervision of healthcare learners supporting their development and the delivery of services to patients of St Marys. To maintain awareness of national and local policies, guidance, and best practice relevant to the role and to adapt practice accordingly. To develop external networks within the PCN, city and more widely to share learning and promote the role of Advanced Practitioners within general practice. To work with the Partners and Practice Senior Management team to contribute to the ongoing development of high quality and sustainable models of primary care.
