Job Description
To provide cognitive testing, assessment, delivery of diagnosis and review in the community/hub. Coordinate and actively participate in the daily running of the Case Management Team. Work collaboratively with peers to co-ordinate the duties and responsibilities of other nursing and support staff within the service on a day-to-day basis. Actively contribute to the development and delivery of therapeutic/treatment plans and psychosocial/educational interventions. Ensure, through supervision, that each allocated service user has a comprehensive, clear and up to date collaborative care plan with specified objectives designed to meet the individuals identified care needs, ensuring effective clear communications and service user/significant others involvement in the formulation, while respecting the wishes and autonomy of the individual Participate and input into the multi-disciplinary team meetings [MDT] providing follow up as necessary/indicated post MDT Identify and make appropriate referrals to other agencies/services to support service users needs Undertake comprehensive nursing assessment of clients using investigative and analytical skills to formulate individual treatment plans. To monitor, evaluate and modify treatment for clients in order to measure progress and ensure effectiveness of intervention. To demonstrate an understanding of the effect of disability and recommend adaptations to the clients social and physical environment. To provide Nursing Services in working conditions which may be hazardous e.g. exposure to bodily fluids, aggressive persons, visits to high-risk areas and associated travel risks. Provide support and guidance to junior staff members To participate in the planning, evaluation and audit of practice, clinical pathways and protocols within the service area. Develop and maintain effective working relationships with Primary Care colleagues and 3rd party sector Participate with colleagues in the promotion of an active learning environment in order to maintain and develop high standards of professional and clinical practice. Promoting opportunities for self, nursing colleagues and support staff to access available education/training. Participate in the development of others through reflective practice, continued professional development and delivery of formal and informal teaching as appropriate. To take responsibility for identifying personal development needs, through attendance and participation in clinical supervision and PDR.
