Job Description
To support and implement care that meets the mental, physical, spiritual, psychological and social needs of the patient, being sensitive to age, culture, race, gender, ethnicity, sexuality and disability. To monitor the effects and side effects of medication in the community, with guidance from relevant professionals, such as the nursing team, consultants and pharmacists. To administer intramuscular injections and monitor the effects of these, with the knowledge of contraindications and side effects, being able to relay accurateinformation to patients in a format that they are able to understand in accordance with SOP0031. To undertake venepuncture in the context of physical health monitoring, after relevant training and ongoing guidance. To undertake physical health checks, interpret results and respond to and report concerns appropriately. (No moving and handling required). Assessment, Care planning and Risk Assessment in line with Trust Policy. To be able to identify and respond to concerns appropriately. The ability to make a judgement about a patients mental health or wellbeing needs and act appropriately to manage needs / risks. (For example, supporting self-care needs by helping a patient to organise their prescribed medication into a dosette box, or reporting a concern / risk to a qualified member of staff). To maintain the safe storage of medical equipment within the clinical setting, in line with Trust Policy. To monitor clinical stock supplies levels and support in the maintenance of adequate stock levels. To take responsibility for maintaining the safe use of clinical equipment used within the role. To contribute to the provision of a safe clinical environment and promote the dignity of patients. To carry out team Audits with support as required, such as the Hand Washing Audit. To prioritise workload based upon the patient needs / service demands. To support multi-disciplinary meetings and contribute to information sharing and clinical decisions regarding service users in the care of the CMHTs. To support patients whose health and wellbeing is at risk, acting upon and reporting concerns appropriately. To have the ability to accurately communicate information to patients in a format that they are able to understand. To demonstrate understanding of the distress and emotional impact caused by mental illness for patients and their carers, as well as the impact upon their social and personal circumstances. To adopt a non-judgemental approach with patients based upon respect and dignity. To assist patients, as required, to obtain support from agencies such as welfare rights, employment support, housing and substance misuse services. To assist patients to develop skills for everyday living i.e. budgeting money, shopping, cooking, establishing or improving social networks, accessing local services etc. To have the ability to identify limitations and to be able to seek assistance and guidance from senior staff as necessary. To ensure the safety, wellbeing and interests of patients, staff and visitors. To have an awareness of and follow Local and National policies and procedures in relation to the clinical role. To demonstrate an understanding of the psychosocial impact that mental illness can have on service users and their carers to be able to promote their wellbeing. To support care programmes, reviewing and modifying care programmes in consultation with the multi-disciplinary team. To support multi-disciplinary meetings and contribute to clinical decisions regarding patients in the care of the Community Mental Health Teams. To work within Pennine Care NHS Foundation Trust policy, professional and legal frameworks at all times. This requires risk assessment skills, awareness of both Pennine Care and Local Authority child and adult safeguarding protocols, relevant mental health legislation and the Care Programme Approach. To keep a high standard of written communication, accurately recording events and observations in accordance with Pennine Care NHS Foundation Trust policies, and the NMC code of professional conduct. To develop and understanding of and demonstrate the skill in the supervision process, using your own supervision and the provision of supervision to junior staff and students. To develop mentorship skills, and to provide professional supervision to and the assessment of student nurses. To account for your own professional practice in accordance with the NMC professional code of conduct, and to maintain the requirements for regular reregistration. To keep up to date with current trends in mental health care government policy and research based practice with particular relevance to this service user group. To recognise and respond appropriately to challenging behaviour in line with Trust policies. To develop and demonstrate skills in the supervision process, both for personal benefit and in supporting junior staff and students.
