Job summary
The Community Matrons Team support housebound patients with multiple health and social care needs to prevent unnecessary hospital admission and provide better health outcomes.
This role is Mon-Fri 9am - 5pm across Westminster.
Main duties of the job
Registered nurse or nurse associate
Current NMC registration
Experience of community health
Experience of monitoring patients with complex conditions and identify deterioration
Ability to undertake vital signs observations including urinalysis, weight, MUST scores
Venepuncture skills
Strong communication, administration and documentation skills
Time management skills
Knowledge and experience of using S1
Ability to work independently and remotely from colleagues
Ability to work across various locations in Westminster (this often requires walking with equipment in trolley bag)

Job responsibilities
This role will be working across the borough of Westminster. Supporting patients on the Community Matrons caseload with:
- Monitoring of vital signs and medication compliance
- Assisting in attending outpatient appointments (inc. escorting)
- Liaison with other health and social care providers
- Identification/signposting of signs of deterioration

Education/Qualification
Essential
- Registered Nurse or Allied Health Professional with regulatory body registration
- Experience of working in Primary Care

- Experience of continued professional development

- Ability to assess and plan individual care needs
- Experience of undertaking clinical observations
- Venepuncture skills
Desirable
- Experience with older frail adults/care homes
- Experience of managing patients with long term conditions
- Ability to lead and motivate a team; work under pressure
- Ability to work across the MDT and social care team
- Ability to handle distressing situations with sensitivity
- IT skills including data collection and analysis
- Experience of using the complex geriatric assessment and urgent care plans
