Job Description

Undertake holistic health assessments of patients with complex palliative and end of life care needs in community settings. To proactively plan, co-ordinate, deliver and evaluate the provision of a high quality specialist palliative and end of life care within the community and across the primary and secondary care interface ensuing onward referrals are made as appropriate. Make appropriate clinical decisions independently and seeking senior guidance where necessary in reviewing, monitoring and managing patients. Assess plan and evaluate care for patients, their families and carers . Monitor patient needs and interventions and appropriately report to the identified key worker. The post holder will provide a therapeutic relationship with patients with a palliative diagnosis, their carers and their families. To provide opportunistic informal education to patients, their families and health professionals. Provide patients and their families /carers with information. Work as part of a multidisciplinary teams acting as patient advocate for specialist palliative and end of life nursing advice and care. They will be expected to work with a wide range of health and social care professionals who work within the acute and community setting. To actively participate in the Palliative and End of Life Care educational programme to deliver evidenced based education to community based staff. Maintain own personal development ,knowledge and skills sharing good practice with colleagues. Actively participate in regular clinical supervision. To develop and sustain effective working partnerships with local clinicians, other specialist palliative care providers, managers and team colleagues. Understand and ensure good practice in managing patient information between the different care settings and teams. Understanding of IT with an ability to use email and other systems that aim to improve communication within the services. Develop and maintain good working relationships with key resources that support service development and influence the role e.g. Macmillan Cancer Support. Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act. Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies. Regular communication with a wide range of colleagues including Primary health care teams Specialist Cancer Care colleagues within Luton, Bedfordshire and Hertfordshire Specialist Palliative Care colleagues within Luton, Bedfordshire and Hertfordshire Other colleagues within the community integrated nursing teams Locality Oncologists, MDT Clinicians and co-ordinators Specialist Cancer and Palliative Care colleagues from tertiary centres Nursing Home Staff Residential Home staff Care Agency staff Hospices and voluntary services Other clinical, managerial and social care staff involved in improving the Palliative Care patients experiencePeople ManagementPromote the use of reflection and retrospective case reviews with aligned primary health care/ care home teams. To ensure a supportive approach to working with the aligned primary health care and care home teams providing:Regular routine face- to- face contact with community nurses and care homes.Prompt response to staff requests for advice/support. To support formal and informal teaching to empower health and social care professionals to deliver a high quality palliative and end of life care service. Maintaining own professional development and requirement to take part in appraisal and KSF process.
