Please be advised that we are expecting a very high number of applicants for this vacancy. Key Responsibilities: 1. Patient Care Coordination (Primary Responsibility) - Develop and review personalised care and support plans with patients, families, and carers. - Proactively identify patients needing coordination, especially those with complex health needs, ensuring they receive appropriate support. - Manage appointments, address queries, and provide information to help patients make informed choices. - Collaborate with GPs, social prescribing link workers, health and wellbeing coaches, and other professionals to ensure cohesive care. - Facilitate communication and follow-ups between providers to support smooth transitions between care services. 2. Multidisciplinary Team (MDT) Support (Primary Responsibility) - Participate in MDT meetings to coordinate care for vulnerable adults and those at risk of emergency hospital admissions. - Serve as a central contact, ensuring care plans are communicated to all relevant professionals. - Refer patients to social prescribing services and other healthcare professionals as needed. 3. Administrative Support (Secondary Responsibility) - Perform general administrative tasks as assigned by the PCN Manager to support PCN operations. - Record meeting minutes, capturing key discussions, decisions, and action points for timely distribution. - Manage patient records with attention to accuracy, confidentiality, and data protection compliance. - Organise meetings, prepare agendas, and coordinate with multiple stakeholders. 4. Patient Support and Advocacy - Support patients in understanding and managing their conditions through tailored information and guidance. - Help patients access self-management education, peer support, and personal health budgets where applicable. - Encourage shared decision-making and help patients prepare for clinical discussions. - Monitor patient progress and outcomes, maintaining accurate records of interventions and their impacts. 5. Administrative and Professional Development - Maintain up-to-date care records using SystmOne and other health platforms. - Engage in ongoing professional development, including role-specific training. - Collaborate with other care coordinators, GPs, and practice staff to advance the PCNs personalised care approach. 6. Key Skills and Competencies - Strong written and verbal communication skills for engaging with diverse individuals. - Excellent organisational and time-management abilities. - Empathetic, patient-centred approach to care. - Accurate recording of meeting minutes with attention to detail. - Ability to work independently and collaboratively within an MDT. - Proficiency in handling administrative tasks and flexibility for a proactive approach. 7. Confidentiality - Uphold patient confidentiality and ensure all information is managed in line with policies and data protection regulations. 8.
