Children's Nurse Assessor

Ensure that the health care needs of children and families eligible for Continuing Care are identified and met in accordance with the National Framework. Support the Lead Specialist Nurse and Continuing Care Strategic Lead in the development of a range of services for children with complex health needs including palliative care services. Undertake specialist nursing assessments for children with complex needs in relation to care packages, respite care and placements for continuing care. Collate and undertake comprehensive and coordinated assessments for decision making for NHS Continuing Care with children and families within health and social care and care plan and commission services to meet the needs of eligible clients. To be instrumental in the development and maintenance of effective systems, procedures and pathways to support Continuing Care and Personal Health Budgets. To promote Personal Health Budgets (PHBs) ensuring integration into continuing care and that they are offered where requested to those clients who meet agreed criteria. Attend the panels were arranged to present own cases and provide clinical information. Case manages continuing care clients ensuring timely reviews are conducted to ensure health and nursing care needs are being met appropriately and to ascertain care packages continue to be effectively commissioned and are cost effective. Assist in maintaining a database for all patients assessed for continuing care and ensure high quality assurance systems are in place. Develop strong working relationships with partner agencies including Acute Hospital Trusts, local authorities, the Independent Sector, and others. Provide professional nursing advice to Acute Hospital Trusts, nursing homes and social services with regards to Continuing Care and Personal Health Budgets. Work with the continuing care team, ensuring that the reviews are carried out. Following the national framework with regards to timelines of assessment and reviews.
