Job Description

Clinical Practice Provide expert nursing assessment and advice to patients attending Day Hospice and Wellbeing Services Provide additional nursing support for outpatient services including complementary therapies and integrated clinics, at Ty Hosbis Implement clinical escalation plans where appropriate Offer support empathy time and reassurance to patients families and carers enabling them to explore and express feelings and issues of concern Identify patient and family goals and work with them towards achieving the Promote patient choice autonomy and independence Working with the Head of Day Hospice and Wellbeing Services develop and deliver an expanded range of patient and family activities including end of life care planning and carer support Co-facilitate or support the delivery of Day Hospice and Wellbeing Services activities such as fatigue and breathlessness management groups with other members of the multi-disciplinary team Work in an integrated way, ensuring seamless support for patients and families across City Hospice, and where appropriate across organisational boundaries Be an advocate for appropriate needs-led care Maintain concise and contemporaneous clinical records Support and build carer confidence in managing a relative whose condition is deteriorating Demonstrate sensitivity and respect for patients and families from different religious and cultural background and sexual orientation Where appropriate bring closure to relationships with patients and/or carers and enable a safe discharge or transition to the wider team Communication Sensitively manage communication issues with patients and families Use clear and unambiguous language to facilitate understanding Anticipate barriers to communication and take timely action to improve the communication skills and strategies of the Day Hospice and Wellbeing Services team including volunteers Implement patient and family feedback mechanisms including one-to-one patient or family interviews or focus groups Communicate effectively in meetings with members of the wider multidisciplinary team Grief Loss and Bereavement Utilise referral pathways to ensure timely onward referral for counselling and bereavement services Where appropriate and/or required effectively manage a single structured bereavement visit for carers following the death of a patient Working with the Head of Day Hospice and Wellbeing Services and the Counselling and Bereavement Team, support or co-facilitate bereavement evenings or remembrance activities As part of the wider City Hospice team, work with colleagues to ensure planned remembrance activities sensitively meet the needs of bereaved families Leadership and Management Work with the Head of Day Hospice and Wellbeing Services to manage day to day service delivery Work collaboratively to design new and bespoke services and activitie Develop and maintain high quality working relationships across the City Hospice clinical team to ensure care is integrated across all services Work with the clinical administration team to ensure effective processes are developed and implemented from referral to bookings and discharge Coordinate and deliver a range of activities for patients and families independently or in collaboration with the City Hospice multi-disciplinary team or external colleagues Provide supervision and support for Day Hospice and Welling Services volunteers including complementary therapists Provide supervision and oversight of students supporting the delivery of Day Hospice and Wellbeing Services Contribute to the development and maintenance of a culture where people feel valued for their contribution and feel clear and confidence in their skills and capabilities Step in for the Head of Day Hospice and Wellbeing Services as required, including at relevant internal and external meetings Work effectively and safely with skilled volunteers, valuing their contribution and being a point of escalation for concerns or issues Education Utilise learning opportunities to enhance the knowledge and skills Initiate and participate in formal training programmes in collaboration with the Lead Clinical Nurse Specialist for Education and Training Support learners undertaking health and social care courses including complementary therapy students Work with the Lead Clinical Nurse Specialist for Education and Training to arrange and participate in induction and mentorship programmes for new members of staff and volunteers Research Use reflective skills to evaluate current practice Identify potential areas for research and engage with the appropriate colleagues to determine feasibility Participate in clinical audit and research as appropriate and apply relevant findings to practice and teaching Professional Work within the Nursing and Midwifery Council Professional Code of Conduct Adhere to legal rules and statutory regulations relating to practice Work within the policies and procedures of City Hospice Participate in clinical supervision and other structures for clinical and professional support Identify own professional development and support needs Participate in the performance review system
