Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care across the Primary Care Network. Overarching goals are to help keep patients safe and well out of hospital and reduce the demands on general practice regards medicine use. The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations processes, while addressing both the public health and social care needs of patients in the GP practices that make up the PCN. The post holder will provide leadership on quality improvement and clinical audit and well as managing some aspects of the Quality and Outcomes Framework. This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice. As well, the post holder will develop and encourage a positive culture of medicines optimisation principles to assist the PCN in achieving local and national medicines-related objectives. Main duties: Help achieve efficiencies and effectiveness within the prescribing activities of the PCN group of GP practices. Promote high quality cost-effective and evidence-based use of medicines within the PCN. Work within the Pharmacy Team, with the pharmacy technicians to respond to medication-related queries generated by patients, GPs, other health care professionals and staff. Work within the Pharmacy Team to ensure a safe efficient medicines optimisation programme including repeat prescribing, medication review and medicines reconciliation services. Become proficient with TPP/SystmOne and EMISGP prescribing system and supporting IT to record personally generated information and maintain a database of information relating to the work done in the practices. Undertake clinical medication reviews either face to face or via telephone support on identified stratified-risk patients that may include residents in care homes, in their own, with long-term conditions, and/or learning disabilities. Identify and produce recommendations for medication optimisation for GPs and other multi-disciplinary team members. Work with care home staff to improve safety of medicines ordering and administration if required. Work to encourage good management of common/minor/self-limiting ailments, including promotion of self-care and homely remedies. Contribute to the safety of drug monitoring processes and drug alerts. Analyse, interpret and present medicines data to highlight issues and risks to support decision making. Advocate and implement local, regional and national formulary recommendations, including public health campaigns. Liaise with all members of the local PCN including community pharmacies and community nursing team, secondary care providers and regional ICB teams on prescribing-related matters. Aim for good collaborative working practices and develop relationships with other professionals across PCNs and the wider health and social care system. Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities. Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety. Duties will vary according to the demands on the PCN as agreed with the PCN Clinical Director.
