Cluster Pharmacist - Cardiff South West Cluster

Main duties of the job To work in collaboration with the Cluster and wider multi-disciplinary team and with voluntary organisations, local authority and other appropriate health and social care sectors to negotiate care provision across the primary/secondary care interface. To support hub staff with patients that have complex needs, using evidence based / client centred principles to assess the patients needs, that may include accompanying on home visits. To maintain clinical records in accordance with legal requirements and professional standards Proactive and contributes to the cluster & hub projects. Demonstrates effective communication skills internal and external to the team and competently disseminates information to a wide-ranging audience. Complete complex hospital discharges for patients within the cluster. Carry out complex medication reviews for NOAC patients. Attend cluster meetings, MDTs, hub meetings, Answering telephone calls, answering emails, complete admin duties within the hub, Maintain up to date standard operating procedures (SOPs) Review bloods for creatinine clearance Polypharmacy reviews Care Home reviews This post will require you to work as part of a team and will require you to have strong IT skills and be confident in undertaking a range of computer-based activities.
