Job Description
To develop and maintain a detailed knowledge of local community and voluntary sector services to enable sign posting of people at risk of loss of independence. Become the practice expert on what's happening in the local community. To develop relationships for the benefit of the practice population with community groups/organisations and health and social care bodies. To provide an ambassadorial function for the practice in its relationships with the voluntary sector, volunteer networks, community groups and all relevant statutory agencies. Working with the practice and community staff, utilising risk stratification information, to identify people at risk of loss of independence or admission as a result of inadequate social support. Where deficits are identified support the community in the development of peer groups, carers groups and volunteering including the compassionate communities movement.
