Job Description

Aim: To co-ordinate all referrals received for all community hospice services ensuring patients receive timely and appropriate care. Professional: 1. Co-ordinate all referrals received for Hospice services. 2. Advise patients, carers and healthcare professionals on the range and scope ofhospice services available. 3. In liaison with the hospice multi-disciplinary team, facilitate efficient and timelyaccess to all services. 4. Provide specialist support and advice to patients, families, hospice staff, volunteersand other healthcare professionals. 5. Where immediate access to services is not possible, explore other sources ofsupport for the patient in liaison with community health and social care teams. 6. Promote and maintain effective communication with external agencies ie GPs,Practice staff, District nursing, Specialist nurses, Acute trust, Adult Services (RMBC). 7. Establish and maintain positive and supportive relationships with all relevantstakeholders. 8. Work closely with the wider multi-disciplinary team to provide expertise, knowledgeand support to patients, families and carers who require support in the community. 9. Promote and take an active part in the Hospice at Home community service,providing the Hospice at Home team with relevant information relating to patientsand ensuring information in respect of service delivery is appropriately communicated to other members of the multi-disciplinary clinical team. 10 Be responsible for the smooth running of the Night Sitting Service in conjunctionwith, Volunteer/Night sitting Co-ordinator and Community Services Manager. Takethe night sit handover following the sit and action as required. 11. Be responsible for the smooth running of the Community Liaison Office on a day today basis. 12. Provide operational support and supervision to the community administration staffin conjunction with the Community and Corporate Services Managers. 13. Promote and take an active part in the 24 hour advice line service, providing adviceline staff with relevant information relating to patients and ensuring informationrelating to telephone calls is appropriately communicated to other healthcare professionals involved in the patients care. 14. Maintain comprehensive documentation relating to patient referrals by use of theHospice patient information system. 15. Ensure close communication, liaison and joint working with members of the clinicalteam within the Hospice 16. Ensure that patient records are maintained and reports produced in accordancewith both statutory and Hospice requirement 17. In line with quality monitoring and governance, to continually evaluate policy,procedure and documentation relating to the role, through relevant sub-committeesand governance groups. Education 1. Ensure attendance at all mandatory training and development sessions required by Springhill Hospice. 2. Contribute to the training and development of other staff within the hospice and provide supervision and support for all learner grades of staff including external students on placement. 3. Take responsibility for identifying own personal and professional development requirements and communicating these to the line manager. 4. To maintain an active role in self-development and continuing education by attending planned educational sessions and development training. Service Improvement 1. Contribute to and participate in organisational service development. 2. Continue to look at ways of developing the Night Sitting Service with the Volunteer/Night sitting Co-ordinator and Community Services Manager. Personnel 1. To read, understand and comply with all Hospice policies and procedures, applying the principles as appropriate to role. 2. To be aware of individual specific responsibilities in respect of the Safeguarding of Vulnerable Adults, in relation to role. 3. To ensure continued support and encouragement to colleagues within the clinical team and the wider Hospice team at all times. 4. To undertake, on occasions, such duties and responsibilities of a reasonable nature, which may be delegated by the Community Service Manager/Director of Clinical Services. This includes supporting the admissions co-ordinator with managing referrals to ensure all referrals are processed in a timely manner, completing audits, quality monitoring, supporting the Hospice at home team. Learning and Development 1. Maintain own professional in order to influence service delivery across the organisation. 2. Actively participate in the Hospice appraisal processes to identify own and others learning needs and take necessary steps in order to reach personal, professional and organisational objectives. 3. Practice in accordance with NMC Code of Conduct and ensure that professional registration is maintained. 4. Work in accordance with Hospice policies to ensure that confidentiality is maintained at all times by self and others. 5. Keep up to date with current palliative care practice and professional development and conduct for self and others. Health, Safety and Security 1. Work in accordance with Hospice policies, procedures and guidelines. 2. Ensure that self and others follow robust risk management processes within the Community Team. 3. Ensure that self and others take reasonable care to avoid injury to themselves or others and co-operate with the Hospice in meeting its statutory requirements in relation to Health and Safety. Quality 1. Participate in research and audit programmes and the development and implementation of subsequent action plans. Equality and Diversity 1. Promote and demonstrate awareness of anti-discriminatory practice in accordance with the organisational policies and current legislation. 2. Demonstrate enhanced knowledge and understanding of organisational objectives relating to equality and diversity. 3. Support patients, visitors and staff who need assistance to exercise their rights. 4. Have a sound understanding of local and national legislation, policies and procedures. 5. Play a key role in establishing an environment and service which strives to reflect the diversity of individual needs. 6. Promote a culture which respects and values diversity. 7. Ensure care within your service is consistent with the five principles of the Mental Capacity Act Code of Practice (2005). 8. Recognise and report behaviour which undermines equality and diversity in accordance with organisational policies and current legislation. Service and Project Management 1. In collaboration with Community Services Manager, Clinical Lead and the Director of Clinical Services identify projects which will improve practice, service delivery and effective use of resources. People Management 1. Lead on planning, recruitment and selection of staff in line with organisational purpose and strategies/Night Sitters in conjunction with Community Services Manager. 2. Support Community Services Manager by undertaking annual appraisal of Volunteer/Night sitting Co-ordinator, night sitters and community administration staff, identifying current and future training and development needs in conjunction with Community Services Manager. 3. In collaboration with the Education Co-ordinator and Community Services Manager contribute to a training needs analysis in order to inform future service development. 4. Take responsibility for the management of staff sickness for staff under your direct responsibility (Volunteer/Night sitting Co-ordinator, night sitters and community administration staff) in the absence of the Community Services Manager. 5. Support the Community team by authorising annual leave and changes to the electronic rota, ensuring safe staffing levels are maintained.
