Job Description
Job Summary This role within First Community (FC) is to provide nursing support to adults and their families. You will deliver skilled nursing care under the direction of the Senior Nurse within the framework of the NMC Code, promoting a healthy lifestyle and independence where possible. You may take responsibility for overseeing the nursing care within your area in the absence of the Senior Nurse. Key responsibilities/outcomes To work within a rota to cover the needs of the service, 52 weeks of the year. To plan and monitor the delivery of person centred health care, evidencing clinical decision making in the clinical record. To participate in the admission and discharging planning of patients from the caseload. To promote the wellbeing, health promotion and self-management of patients health needs. To work within FC and local policies, guidelines, procedures and SOPs. To promote and ensure effective communication, working within FCs behaviour framework. To supervise and delegate care provided by more junior staff, while maintaining accountability. To recognise and escalate any concerns to the senior nurse. To maintain records in accordance with FCs policy and NMC Guidelines, including electronic patient and data management systems (EMIS). You will be able to work independently and as a member of the team, working in partnership with the wider Health and Social Care Networks. To maintain own professional and clinical integrity in line with the NMC Code. To effectively engage with all team members concerned with patient care. To support the induction, supervision and teaching of junior team members, including student nurses and temporary staff. To be an advocate and role model within the community setting. To be aware of budgetary constraints and effective resource management. To ensure that local and national Health and Safety regulations are adhered to by all members of the team. To report and escalate any risks and follow FCs incident reporting process. Be aware and act upon procedures that are in place to protect vulnerable individuals. To attend and engage in organisational meetings and feedback to the team. To take an active part in clinical supervision and reflective practice sessions. To assist in service audits where indicated, supporting the continued review of effective and efficient practice.
