Salary: Starting salary £193.25 per session Tenure: 31st March 2027 Hours: Two sessions per week Location: Predominantly a F2F role based in Scarborough at The Prospect Hub (Scarborough Extended Access Hub) with visits in the surrounding Scarborough, Whitby & Ryedale area. The GPwER in Frailty will provide evidence-based, comprehensive, person centred and individualised medical care for older people, working as part of a multidisciplinary team within community settings across Scarborough, Ryedale and Whitby community. This will include patients referred to the Virtual Ward and across other community services, including the Urgent Community Response Service, as part of holistic patient clinical pathway management You will need to hold the relevant professional registration (GMC) and have experience of Triage and Telephone Assessment. Experience of working in Geriatric Medicine is desirable. Any successful candidates would join our growing team of GPwERs in Frailty and be supported to obtain their diploma in Geriatric medicine as well as join our regular education sessions. F2F Predominantly a F2F role based in Scarborough at The Prospect Hub, Scarborough Extended Access Hub, with visits in the surrounding Scarborough, Whitby & Ryedale area when required the clinician will be responsible for making autonomous clinical decisions, assessing the complex needs of patients on the VFW, be responsible for the clinical management plans for the patients with opportunities for involvement in service development with the clinical lead as appropriate The post holder will work as part of a multidisciplinary team, which includes Consultant Geriatricians, GPwERs, nurses, therapists, social care, the third sector and care home staff, to deliver high quality care and support for older people within the community, including completion of a Comprehensive Geriatric Assessment where clinically indicated The post holder will be expected to work autonomously but will also have supervision and clinical mentoring provided, as well as engage in peer supervision Roles and Responsibilities Communication To communicate complex information regarding personal, psycho-social problems in an empathic and supportive way. This will involve using a range of developed inter-personal skills in situations that can be emotional, and or antagonistic and require a satisfactory outcome, e.g. impact of diagnosis. To be able to communicate with all health and social care professionals verbally, and through written and electronic platforms, to ensure effective, responsive, and safe patient care The post-holder should recognize the importance of effective communication within the team and will strive to: - Communicate effectively with other team members. - Communicate effectively with patients and carers. - Recognise peoples needs for alternative methods of communication and respond accordingly. - Communicate with wider MDT including patient's own GP practice. Clinical Responsibilities To have confidence and abilities in providing holistic assessments and management plans and making sound clinical decisions with confidence to escalate when needed. The post holder will significantly contribute to advance care planning, and Treatment Escalation Plans through completion of CGAs and ReSPECT forms, for patients within their clinical care The post holder will be expected to practice within the policies, procedures and guidelines of the organization and service, and national bodies such as NICE, The Royal College of General Practice. Must practice within the ethical boundaries of Good Medical Practice as laid down by the GMC. Operational, Governance and Delivery Proficient in using relevant IT systems including SystmOne, Microsoft programmes and remote clinical tools. The post holder will be responsible for the development and safe storage of person centred health care assessments, plans and electronic care records. The post holder may also be responsible for the managing and safe storage of electronic staff records, for example clinical supervision records. Maintain up to date statutory and mandatory training and CPD Awareness and compliance with all relevant guidelines e.g. data protection, confidentiality and health and safety Maintain current CPR and anaphylaxis certification and child and adult safeguarding training in line with local policies. Complies with all relevant clinical governance and Practice polices and protocols. Records data in patient records systems promptly and accurately and to agreed standards. Ensures appropriate use of read codes Contributes to the evaluation and audit and clinical standard setting with colleagues Ensures that the complaints process is adhered to at all times. Fully participates in training and development, meeting any, and all, requirements of any courses funded or part-funded by the service. Responsible for health and safety of themselves and others and for reporting and actively co-operating in Health and Safety Policies and procedures. Attends in-house meetings, significant event audit meetings and other meetings as deemed necessary. Responsibilities for Patient Care Making professionally autonomous decisions in relation to presenting problems, whether self-referred or referred internally within the Practice. Conduct virtual or F2F appointments with patients on home visits or via telephone or online mechanisms. Undertakes triage calls, triage visits, checking and signing of repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion as required. Compiles and issues computer-generated acute and repeat prescriptions, prescribing in accordance with the ICB prescribing formulary whenever this is clinically appropriate. Admits or discharges patients to and from caseload and refers them to other care providers internally and externally as appropriate and in line with practice protocols. Assists in the establishment of appropriate systems to manage common chronic medical conditions.
