Health promotions and communications Coordinator
Job Description
The health promotion and communications element of the role will include: Coordination of patient engagement activities across the PCN including public facing organisation of events and health promotional activity Leading and evolving the patient and public engagement via the Patient and Public Engagement Group (PPG) in partnership with the PCN Director and Clinical Directors Overarching coordination and day to day management/optimisation of PCN and practice websites Overarching coordination and day to day management/optimisation of social media platforms Coordination and management of public facing PCN newsletters optimising all opportunities to engage Optimising relationships across local partners to ensure patients and their families are optimising local opportunities/services Support PCN practices to ensure relevant health promotional activities are maximised Care Coordinators will: Support Quality and Outcome frameworks and other National targets as required. Work collaboratively with other teams and services to maintain an effective and efficient service. Be responsible for the collation of Covid vaccine orders and stock take of vaccines and the delivery of those vaccines to our practices. Plan / organise work using own initiative, whilst being able to work as a valuable member of a team. Add appointment templates and supervision slots (if required) for all PCN clinical staff. Use a range of verbal and non-verbal communication tools to communicate effectively with patients, carers, families and colleagues. Complete mandatory training. Participate in the review and appraisal process. Use clinical systems for record keeping, audit and evaluation. Provide support with digital transformation and offer IT/System support across our sites. Ensure all social media pages are up-to-date, informative and engaging. Build key relationships across PCN personnel and the wider team Provide coordination and navigation for people and their carers across health and care services, alongside Social Prescribing link workers and other primary care roles; Support the coordination and delivery of MDTs within PCNs as required; Focus upon patients and provide support necessary fulfil effective personalised care requirements; Help patients to manage their needs, answering their queries and supporting them to make appointments with a focus upon promoting patient independence and autonomy wherever possible; Coordination of key meetings (including Care Home residents and frail/vulnerable people living within their own homes. Ensure good communication between professionals and be available as a point of contact between partners; Liaise with multi agencies to coordinate pathways of care for patients. Support Quality and Outcome Frameworks and other National and Local targets as required.
