Registrar ST 3 - 5 fixed term post for Hospital at Home Services
An exciting opportunity has arisen for a fixed term registrar (ST3 or above) to join our dynamic, friendly and growing Hospital at Home (H@H) and Acute Medicine services at Dartford and Gravesham NHS Trust. The fixed term is for an initial period of 6 months extendable to 12 months.
We are pleased to announce a unique and highly rewarding opportunity for a resident doctor at ST3+ level in Higher Specialist Training to join the Hospital at Home (H@H) and Same Day Emergency Care (SDEC) services at Darent Valley Hospital. This newly created post is part of a pioneering cross-sector expansion initiative designed to enhance our established care pathways. Main duties of the job
The aim of SDEC, H@H and virtual wards is to achieve a reduction of avoidable hospital admissions and to facilitate earlier, safe discharge of patients from the acute services.
Further to this, there is substantial opportunity to work with the clinical lead to identify additional service development, lead on change and provide leadership for junior colleagues and the acute teams. Job plans are designed to facilitate this.
A full-time job plan with working hours from 8.30 - 16.30. You will be expected to triage referrals, review patients on the take that may be managed directly under Hospital at Home rather than being admitted to an acute bed, and supporting the step down of patients from the ward. You will be involved in reviewing patients as well as formulating management plans for patients under hospital at Home. You will occasionally accompany nursing staff to visit patients in their own home. You will work seamlessly between SDEC and HAH. We expect the post-holder to be involved in the development of medical students, trainees and allied health professionals.

Job responsibilities
Successful implementation of the Same Day Emergency Care Unit in the Trusts newly developed SDEC Village alongside the Frailty Assessment Unit and H@H ensures that timely care is delivered whilst avoiding unnecessary admission, reducing duplication and ensuring that sickest patients are prioritised.

Person Specification
Qualifications
Essential
- MBBS or equivalent
- MRCP Full
- Full Registration with the GMC
- At least 6 months experience in a medical speciality at a higher level (eg SpR)
Desirable
- At least 12 months experience in acute general medicine
- Middle grade experience working in acute medicine
Knowledge & Skills
Essential
- oExperience in relevant procedures, including: ascitic drains, chest drains and Lumbar Puncture
- oKnowledge of the Hospital at Home Operational Framework
- oUnderstand the principles of patient confidentiality and data protection
Desirable
- oExperience in ultrasound
- oSpecialist interest relevant to acute medicine or front door medicine
- oExperience and interest in Quality improvement and management
Experience
Essential
- oAt least 12 months experience in acute general medicine
- oMiddle grade experience working in acute medicine
- oEvidence of participation in audit and clinical governance
Desirable
- oExperience in working in community services such as primary care
