Job Description

Conduct an assessment of need, risk and motivation and make onward referrals to specialist services Act as a key worker whilst maintaining a high standard of records and case notes using the electronic system Provide service users and their families with information and advice verbally and through providing relevant literature Engage potential service users and signpost to treatment services and other relevant agencies within the locality Create an individually tailored package of care, in line with statutory requirements. Liaise with other health and social care professionals where necessary and appropriate To offer brief interventions, psychosocial interventions and harm reduction strategies To communicate highly sensitive information within an often hostile and emotive environment to service users, relatives and colleagues. To undertake screening for Drug and Alcohol To devise and review recovery care plans to ensure that service users needs are met in a timely and appropriate manner Function as part of a multi-disciplinary team, contributing to team meetings as required. Understand and use of pathways available to substance misuse service users Identify problems and issues and support service users to find a solution To assist in the decision-making process regarding the referral of service users children to the relevant safeguarding agency assess and interpret complex and conflicting information Take personal responsibility for maintaining practice in line with developments and new initiatives. Collate statistical information as required Proactively encourage drug and alcohol users to access substance misuse services. To maintain and update service users records, ensuring the confidentiality of such records is in line with the GDPR and any information sharing protocols in place. To report as soon as is practicable any incidents Undertake diary management, resolving conflicting appointments and schedules as required, which involve arranging appointments and meetings, ensuring that appropriate communications are actioned and documents are available. Positively promote the service during internal and external meetings
