Job Description
The Enhanced support Team work 7 days a week to support people with learning disabilities during episodes of mental health or behaviour crisis to reduce the need for admission to hospital. This will include the following: - 1. The post holder will have the clinical skills relevant to the delivery of care in the service area or demonstrate an aptitude to develop these skills. 2. The post holder will ensure compliance with their relevant clinical bodys codes conduct and practice, relevant legislation, policies and procedures. 3. The post holder will take responsibility for formulation of appropriate care plans for those service users presenting with challenging behaviour or mental health crisis. 4. The post holder will be able to assess service users thoroughly, plan care rigorously, audit, and evaluate interventions based on evidence and locally agreed protocols and policies. 5. The post holder will act in a consultative role to external agencies in relation to clinical interventions, risk assessments and management plans. 6. The post holder will ensure they are fully conversant with all aspects regarding the service user care and treatment. 7. The post holder will offer professional support to junior members of the Enhanced Support Team in accordance with professional responsibility. 8. The post holder will provide interventions in a person centred way that promotes and facilitates the positive health and wellbeing of people with learning disabilities. 9. The post holder will maintain close liaison with all departments involved in a service users care and report on progress and effectiveness to the MDT. 10. The post holder will participate in reviews within the community and inpatient settings and will prepare reports on service users as required. 11. The post holder will be aware of all policy documents and legal requirements pertinent to the post and ensure that these are followed. 12. The post holder will demonstrate and serve as an example for clinical activities to appropriate grades of staff. 13. The post holder will lead specific groups and sessions with clear objectives. 14. The post holder will ensure that the statutory requirements of the Mental Capacity Act and Mental Health Act are observed and applied. 15. The post holder will provide specialist education and training regarding learning disability for those people working within local health care/social care/residential care/third sector settings. 16. The post holder will carry out clinical procedures when required in accordance with trust policy and their own competency. 17. The post holder will act as a practice assessor and practice supervisor to students. 18. The post holder will provide induction and training of others. 19. The post holder will undertake clinical risk assessment and develop with these with comprehensive multi-agency contingency planning. 20. The post holder will provide clinical advise for service users who present with challenging needs. 21. The post holder will carry out joint assessments with other MDT members. 22. The post holder will support in the development of evidence based quality practice. 23. The post holder will take part in research in the field of challenging needs and keep up to date. 24. The post holder will provide training in response to specific service user need. 25. The post holder will administer medication, where appropriate, in accordance with professional, legal and local requirements. 26. The post holder will be support service user needs led meetings as part of holistic care planning. 27. The post holder will communicate clinical formulations, hypotheses, current problems, contributing factors to service users, families, advocates, care providers and other stakeholders. 28. The post holder will play an active role in representing the EST in the ongoing development of the service and relevant care pathways. 29. The post holder will support in developing working partnerships with inpatient services and primary secondary and tertiary care. 30. The post holder will provide clinical supervision for junior staff. 31. The post holder will monitor delegated tasks to unqualified staff as part of direct and indirect supervision. 32. The post holder will contribute to the orientation and induction programmes for new staff and also to their training and development. 33. The post holder will promote the inclusion of people with learning disabilities into mainstream services ensuring that they have access to and are included in service development and improvement planning. 34. The post holder will be able to analyse risk assessment information and transpose using unambiguous everyday language into a risk management plan. 35. The post holder will be able to apply rational judgement and maintain sound clinical decision making skills within areas of responsibility in crisis situations.
