Key functions of the role: Your responsibilities include assessing patient referrals planning implementing and evaluating care in patients home managing a community-based caseload and ensuring personalised compassionate palliative care To lead our specialist palliative care nursing and AHP team to fulfil their roles and our strategic aims and work with members of the primary health care team in the community acting as a specialist resource To provide direct support to the Consultant in Palliative Medicine in improving internal and external education and training and to develop and sustain specialist palliative care knowledge and skills within the MDT To work closely with and to develop professional relationships with health care professionals and other palliative care teams within hospitals the community and hospices To act as a role model demonstrate excellent clinical skills and judgement and uphold the charity's values Communication and Relationship Skills Lead effective communication within the multidisciplinary team ensuring that complex information regarding patient care symptom management and treatment plans is clearly conveyed to all stakeholders Effectively reports and analyses information regarding patient condition and areas of complex symptom control Develop and maintain professional relationships with primary healthcare teams palliative care teams and other healthcare providers within the community and hospitals to ensure coordinated and seamless palliative care Develop and maintain professional relationships across the charity to fulfil our mission and achieve the City Hospice strategy recognising the value of community engagement and raising the profile of the charity's work Advocate for patients by representing their needs and preferences in MDT meetings and using negotiation skills to secure the best possible outcomes Ensure accurate and timely documentation and sharing of patient information across the MDT utilising robust data management and IT skills to support these processes Ensure effective systems are in place to enable patient experience feedback and collect data on patient outcome measures Ensure that all members of the MDT maintain accurate and up to date records and review asper professional and legal requirements Use excellent negotiation skills to liaise with patientcarers families and outside agencies involved with palliative care Embrace the charitable ethos of City Hospice acting as an ambassador for the organisation and supporting volunteer and fundraising activities as appropriate Knowledge Training, and Experience Ensure continuous professional development for yourself and your team staying current with best practices in palliative care and maintaining up-to-date qualifications relevant to the role including a Masters degree and a professional teaching qualification or suitable leadership qualification (such as ILM or equivalent) Lead and oversee the application of specialist knowledge in clinical risk management ethics in practice and nursing law ensuring these are consistently applied across the team Apply own specialist knowledge and experience of oncology and / or working with terminal non-cancer patients and / or palliative care Experience of overseeing and involvement with clinical risk management Experience of ethics in clinical practice and nursing law in practice Analytical and Judgemental Skills Apply advanced clinical reasoning and problem-solving skills underpinned by theory evidence-based practice and continued professional development to assess plan implement, and evaluate complex patient care needs Assumes professional autonomy responsibility and accountability to implement palliative care Applies clinical reasoning and problem solving techniques Identifies the need for further assessment and appropriate intervention Make autonomous decisions in the management of patient care balancing clinical and ethical considerations to ensure the best outcomes which enable the patient to maximise function promote independence and maximise quality of life Manage challenging situations with skilled conflict resolution and negotiation ensuring that patient care remains the priority Prioritise workload and take responsibility for management of self and caseload Use standardised and non-standardised assessment tools to identify physical and functional needs in the areas of patient management Accurately record assessment method outcome professional judgement and decisions taken From the results of the preliminary assessments and in consultation with the patient identifies the need for further assessment appropriate intervention and/or negotiates with other services/agencies to meet additional care needs Planning and Organisational Skills Lead the planning and organisation of clinical services to deliver high-quality palliative care within set budgets and targets Prioritise workload effectively balancing leadership and managerial responsibilities with clinical care Develop and implement systems and processes to ensure the efficient delivery of car, including participating in and leading case conferences within the MDT Take a leading role (in conjunction with the Clinical Director) in the strategic planning of service development identifying and implementing improvements in care delivery sustaining quality and considerate of the patient and family experience Responsibilities for Patient Care Provide specialist complex palliative care to patients offering complex physical emotional and psychological support with regard to the palliative aspects of diagnosis/treatment surgery radiotherapy and chemotherapy To advise and educate patients and their families/carers on the management of individual problems incorporating individual patient focused goals and outcomes Manage clinical risk within your own patient caseload ensuring that care plans are safe effective and responsive to changing patient needs and those of the MDT Uphold the highest stand and personal beliefs and ensuring their needs and choices are central to all palliative care decisions Responsibilities for Policy and Service Development Implementation Contribute to the development and implementation of nursing policies and clinical standards in accordance with the NMC Scope of Professional Practice and City Hospices policies and strategic goals Support the delivery of decisions agreed at Clinical Governance Committee proceedings Critically appraise and report on the impact of palliative care services in the community identifying areas for improvement and development Work with senior staff to ensure that service development initiatives are aligned with best practices and organisational objectives supporting the continuous improvement of City Hospice services Responsibilities for Financial and Physical Resources The postholder will ensure the optimisation and cost effective use of all resources and will have a direct input into the budget planning process Participate in the budget planning process for clinical services ensuring the cost-effective use of resources whilst maintaining high standards of care Manage the safe use of equipment and resources required for patient care Monitor and optimise the allocation of resources ensuring that financial constraints do not compromise the quality of care provided Responsibilities for Human Resources (HR) Line manage the CNS Palliative Care Nurse team and other AHPs including conducting appraisals identifying training and development needs creating personal development plans and contributing towards the charity's strategic objectives Facilitate and participate in clinical supervision and the training of students on clinical placements ensuring that all team members receive the support and guidance they need to excel in their roles Ensure compliance with statutory and mandatory training requirements Contribute to the development and delivery of in-service training programmes ensuring that all staff are up-to-date with statutory and mandatory training requirements Manage and participate in City Hospice service reviews helping to identify and implement strategies for service improvement and staff development Undertake regular and appropriate continuous professional development (CPD) activity to maintain knowledge and skills relevant to the role of Lead CNS The postholder will promote an environment conducive to the training needs of staff and participate in teaching Freedom to Act Professional accountability to the Clinical Director and Consultant in Palliative Medicine being accountable for clinical decisions and actions whilst working within City Hospices policies and procedures Work in accordance with National Guidelines of NICE and relevant government policies ensuring that all clinical practice is evidence-based and aligned with best practices Report directly to the Clinical Director and Consultant in Palliative Medicine seeking guidance and support when necessary Maintain high levels of concentration and focus particularly when assessing complex cases or managing competing demands on your time Respond effectively to frequent interruptions and unpredictable events such as changes in a patients condition whilst maintaining a high standard of care Balance the demands of leadership and clinical responsibilities ensuring that both are managed effectively Manage the emotional impact of working with patients and families facing life-limiting illnesses providing support during distressing or difficult situations Utilise reflective practices to manage your own emotional well-being and support the emotional resilience of
