Job Description

To assist and perform routine nursing tasks related to patient care, such as blood pressure checks, routine diabetic foot checks, inhaler technique checks, immunisations, BMI lifestyle and advice, as directed and agreed with line manager, registered general nurses and GPs. To support patients to improve and maintain their mental, physical, behavioural health and well being by using behaviour change interventions, such as undertaking health checks (physical and mental), smoking cessation services. Following training and competency assessment, to undertake adult and child NHS immunisation programmes and ensure vaccines/injections are administrated under the appropriate legal framework To undertake administration of prescribed medicines via IM, SC routes, following competency based training and practice policy. To support health action planning, as part of Learning Disability and Severe Mental Illness physical health checks, with the support of practice clinical leads and community specialist teams. To undertake the collection of pathological & microbiology specimens including venepuncture, swabs, urine culture in line with local guidelines To support chronic disease management including monitoring patients with stable asthma, COPD and undertaking preliminary investigations for Type 2 diabetes patients and heart disease, such as urinalysis, arranging ECGs To provide general health promotion education, advice on national screening programs and guide/refer patients to appropriate services. To undertake any investigatory test, such as Ambulatory BP monitoring, automated Dopplers and Spirometry, taking basic history and referring patients to relevant clinician for interpretation/ planning of care. To be able to provide appropriate self care advice for patients presenting with minor illness within own level of competency and referring to GP/ANP where appropriate. Chaperone and assist patients where appropriate who are being examined by another clinician. To maintain effective infection control measures in line with local guidelines and practice policy. To be able to recognise and manage anaphylaxis, according to the current UK Resuscitation Council guidelines. To be able to perform Cardio-pulmonary resuscitation (BLS) and the correct use of Automated Defibrillator following training, according to the current UK Resuscitation Council guidelines. To participate in developments within the practice / network and support QOF / DES requirements. To be registered with the NMC. Work in accordance with the NMC Code of Conduct and Scope of Practice for Nursing Associate Ensure accurate notes of all consultations and treatments are recorded in the patients notes and on computers, as per NMC guidelines Attend and actively participate in practice and network meetings as required. Awareness of statutory protection procedures including referral and recognition of signs and symptoms of child abuse. Awareness of referral mechanisms for mental health issues, family violence, vulnerable adults and addictive behaviours. Actively take part in mandatory and internalexternal training as requested. Adhere to and follow all practice policies and protocols Stakeholder Relationships Work as part of the wider holistic team to provide cover and support as necessary To work with our clinical and digital system colleagues to implement and operate technology solutions which may include wearables or equipment to enable self-taking of health diagnostics e.g. blood pressure, weight etc. Build and maintain relationships with primary, secondary, community and voluntary leads and members of the local support team including named GPs, pharmacist, community nursing team, therapists, mental health professionals etc. Collaborative working relationships: Work within the primary care team, contributing to leadership of service evaluation to promote quality improvement activity. Work in partnership with members of the PCN workforce including medical, nursing, pharmacy, PCN care co-ordinators, link worker, health coaches to personalise care Management: Use resources effectively to manage clinic delivery in line with local and local guidance and makes recommendations for change where improvements can be made. Follows professional and organisational policies Education, learning and development: Comply with all organisational and statutory requirements ( health and safety, infection control, equality and diversity, confidentiality, safeguarding adults and children, information governance). Engages in developing objectives to inform a Personal Development Plan, Supports practice staff and responds to requests for advice and assistance. Complete all mandatory and statutory training required by the role. Takes responsibility for personal development, learning and performance Undertakes additional training where necessary to provide enhanced services and participate in training programmes implemented by the PCN/practices as part of this employment. Understands and demonstrates the characteristics of a role model to members in the team and/or service. Demonstrates an understanding of current educational policies relevant to working areas of practice and keeps up to date with relevant clinical practice. Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development. To engage in clinical supervision Quality: Under supervision and support of GPs, registered nurses and the wider team, the post-holder will strive to maintain quality within the PCN, and will: Participate in clinical governance activity and contribute to the improvement in quality of health outcomes through audit, risk management and Quality Improvement Alerts other team members to concerns about risk, quality, and safety Participates in investigation of incidents and events as required Identify, applies, and disseminates research findings relating to current practice Collect data for audit purposes and uses clinical audit to monitor quality in the service Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team performance Work effectively with individuals in other agencies to meet patient needs Effectively manage own time, workload, and resources Equality and Diversity: The post-holder will support the equality, diversity and rights of patients, carers and colleagues, to include: o Act in a way that recognises the importance of peoples rights, interpreting them in a way that is consistent with Practices procedures and policies, and current legislation. o Respect the privacy, dignity, needs and beliefs of patients, carers and colleagues. o Behave in a manner which is welcoming to and of the individual, is non-judgmental and respects their circumstances, feeling priorities and rights. Confidentiality As per both Government legislation and Practice policies ensure that all confidentiality, data protection and information governance policies and guidelines are followed and strictly adhered to, reporting any infringements to the Management Team immediately.
