Job Description

Main Purpose of Job: To provide high standards of patient care, promoting health, and improving outcomes through collaboration, innovation, and ongoing development. This role offers an exciting opportunity for a dedicated GP to make a meaningful impact on patient care while contributing to the broader goals of the NHS in improving population health outcomes. Location: Newgate Medical Group, The Health Centre, Newgate Street, Worksop, Notts S80 1HP Benefits: Competitive NHS Salary including the NHS pension scheme. 6 weeks holiday & study days (pro rata for part time). Main tasks of job: Clinical responsibilities: 1. Patient Consultations Conduct face-to-face consultations, providing assessments, diagnoses, treatment plans. See 12 patients per session, with 15-minute appointments. Manage acute and chronic conditions, prescribe safely, and handle administration tasks, including referrals and prescription reviews. Maintain accurate clinical records. 2. Chronic Disease Management Managelong-term conditions like diabetes, asthma, and COPD. Work with nurses and healthcare professionals to deliver care plans for chronic conditions. 3. Preventative Care & Screening Promote immunisations, screenings, and lifestyle interventions. Focus on early intervention, disease prevention, and health education. 4. Referral & Coordination of Care Refer patients to specialist services and coordinate care across primary, secondary, and tertiary sectors. Collaborate with multi-disciplinary teams to provide seamless care for complex cases. 5. Health Promotion Engage in health promotion initiatives such as mental health awareness, smoking cessation, and weight management. Refer patients to relevant agencies and contribute to population health strategies aimed at reducing health inequalities. 6. Public Health & Community Engagement Participate in public health initiatives to address social health determinants and inequalities. Collaborate with local services and community organisations to improve patient well-being. 7. Quality Improvement Take part in audits, research, and quality improvement projects. Ensure adherence to appraisal and revalidation processes, promoting continuous professional development. Contribute to reviews of significant events and near-misses using structured approaches like root cause analysis. Foster a culture of collaborative working and continuous improvement across the PCN.
