This role is responsible for working towards our annual campaigns and ensuring we are meeting the required Quality Outcome Framework (QOF) targets for chronic diseases. In addition, you will be expected to take part in the extended hours rota for the Practice. In Accordance with the NMC code of conduct the post holder will assess, plan, develop, implement and evaluate nursing care to promote health and wellbeing, and prevent adverse effects. They will be responsible for undertaking relevant training and ensuring their own competence, with the support of the Practice. Key Responsibilities: Treatment room duties to include venepuncture, wound care, administration of injections and immunisations, carrying out ECGs, removal of sutures and staples, advice on ear care and carrying out ear irrigation where appropriate, urine sampling. Support and supervise trainees with treatment room duties in accordance with their competencies. Carry out cervical screening where postholder holds a current smear takers code. Provide reviews for patients with long term conditions based on QOF. This may include diabetes, hypertension, cardiovascular disease, chronic kidney disease, learning disability and mental health reviews. Provide high-quality nursing care and support to patients with diabetes. Conduct diabetes clinics, including assessments, education, and management plans with the support of the Diabetes Nurse Lead and GP Lead. Collaborate with GPs and other Healthcare professionals to deliver integrated care. Participate in health promotion activities and patient education. Integrate health promotion and patient education within annual reviews. Maintain accurate and up-to-date patient records using EMIS.
