Solutions4Health are delighted to be offering the role of Project Support Officer in Reading. This is a full-time Fixed Term contract job with a salary of £24,960 per annum. Role Summary Solutions4 Health has been commissioned by West Berkshire Council to deliver a community outreach service in West Berkshire, the role in question will require the successful candidate to help with various tasks to support the project. MAIN DUTIES AND RESPONSIBILITIES Uploading client referrals onto the IHS Database Contacting referrals via telephone & email Booking clients onto available clinics Plan and organise workload effectively and be able to work well under a strict timeline. Ensure a smooth referral pathway is put in place and overcome barriers to receiving referrals. Ensure all wellness coaches are fully equipped to run their sessions. Website - Take responsibility for the programme including writing new articles, adding or altering clinics and their opening times, and updating with relevant events and campaigns. Manage venue bookings Increase service awareness in the community and social media platforms Manage all referrals coming into the service via self-referral route and health professionals Motivate and empower clients to make behaviour changes. Run reports regularly when requested Monitor outcomes of treatment programmes for their effectiveness. Develop and maintain close working relationships with key partners To actively participate in promotional events to increase referrals to the service To carry out any other duties relevant to the post. Make sure all client data are entered into the IHS database Assisting with creating and sending out promotional materials for the programme Any other administrative tasks Knowledge and Skills Knowledge of behaviour change models Knowledge of the problems experienced by socially disadvantaged sectors of the community An understanding of the impact of obesity on health Working knowledge of the issues related to information governance Working knowledge regarding safeguarding processes and policies Excellent communication skills Excellent time management and organisation skills The ability to work independently as part of a team The commitment to provide an efficient and effective service Knowledge of Microsoft Knowledge of reporting to commissioners and stakeholders Experience Experience in assisting with public health programmes Working with clients with complex issues Experience in working with clients from a variety of social and cultural backgrounds Experience in partnership working Other Enthusiastic and self-motivated The ability to adopt a flexible working pattern About Us Solutions4Health are an innovative company with a passion and a track record for tackling health inequalities.
