Senior Community Support Worker (Access Facilitator)

Detailed job description and main responsibilities REQUIREMENTS In order to be a successful Senior Access Facilitator in our team you will be: NVQ Level 4 in care, or equivalent qualification orlevel of experience Experience of administering physical health checks and associated interventions Including venepuncture. Experience of working with service users, carers and family members Excellent interpersonal skills Knowledge of physical/mental health conditions and health inequalities for people with SMI (included in the NHS Long Term plan).
