We have a vacancy to join our Frailty Interface Team as a Band 3 Senior Heath Care Support Worker within OPM.

Main duties of the job

As part of a highly skilled and well-established team you will support the Older Persons Specialist Nurses with assessments of frail older patients attending the Emergency Department, Acute Medical Unit and Older Persons Medicine Same Day Emergency Care Unit on QAH site.

The role provides an opportunity to build skills and knowledge of frailty and functional assessments whilst gaining understanding of treatment and discharge planning.

Excellent written and verbal communication skills are essential for this role and paramount when consulting with patients and relatives, as well as liaising with both internal and external healthcare providers.

Job responsibilities

Confidence in your ability to identify and escalate concerns is paramount to this position.

You will work in collaboration with multi-disciplinary teams leading on the improvement, expansion and development of the frailty pathway.

Person Specification

Qualifications

Essential

  • English and Maths at Level 2 Functional Skills or GCSE equivalent.
  • QCF or BTEC or equivalent (NVQ) Level 2 qualification in Health and Social Care/Clinical Health, or completion of NHSP Care Support Worker Development Programme with 1 year experience as a Health Care Support Worker.

Experience

Essential

  • 1 year care experience in an acute health setting.

Desirable

  • Able to demonstrate knowledge / understanding of Frailty / community services
  • Experience of working with people in a caring setting and a multidisciplinary. team including elderly medicine and therapy.
  • To have 1 year as a band 2 in rehabilitation or equivalent setting, such acute stroke or orthopaedics.
