Job summary
An opportunity has arisen for a Social Prescribing Link Worker to join our team and support PCCO Primary Care Network that includes Lister House and Lister House Chellaston with approximately 48,000 patients across Derby.
We are looking for a compassionate, proactive team worker who wants to make a positive difference to the local community in Derby City. Working hours are flexible and open to discussion at interview. This role is up to 37.5 hours per week.
Social Prescribing aims to increase peoples active involvement in their local communities. It can strengthen resilience at both a personal and community level, and it reduces health inequalities by supporting people to address the wider determinants of health, such as debt, poor housing and physical inactivity. It works particularly well for people with long-term conditions (including support for mental health), for people who are lonely or isolated, and those with complex social needs which affect their wellbeing
One of our most desired additional skills for this role would be the ability to engage with our diverse population who face significant health inequalities. This includes the potential ability to speak a second language, ideally Slovak or Slovak Roma, with advantages of speaking Urdu, Punjabi or Czech, among others.
Main duties of the job
The role will involve coproducing a "Social Prescription" with individuals referred to the service which provides the individual with positive health and wellbeing outcomes through addressing non-medical/social issues in their life, such as housing, debt or social isolation. You will be working with two other social prescribers, two health and wellbeing coaches and two care coordinators within our Wellbeing Team.

Qualifications
Essential
- Must be qualified to GCSE or above
Desirable
- Degree level / similar health or care discipline
- Other degree with transferable skills
Experience

- Experience working with people on a One-to-One basis within groups
- Background of helping others
Desirable
- NHS Experience, health care provider or community service
- Client / coaching work
- Social Prescribing experience
- Experience working within Primary Care
- Knowledge of the Network DES Contract

- Ability to travel around Derby efficiently
- Good computer skills and use of Microsoft Word, Excel, Powerpoint and Teams
- Ability to work across multiple agencies in health and social care

- Current clean, full UK driving licence
- Skilful in networking
- Experience in giving health advice to individuals
