Job Description Job Title: Locality Visiting Nurse Practitioner / Emergency Care Practitioner Salary Scale:TBC depending on experience and qualifications Hours: TBC over three/four days Accountable to: The Partners and Business Manager Responsible to: Business Manager Contract: Substantive Job Summary The role will be based across multiple sites focussing on the oversight of patients living in care homes and their own homes in the locality. The post holder will be responsible for supporting the GPs with the management of frail and elderly patients, including first contact interventions. The post holder will take a comprehensive history, clinical assessment, treatment and evaluation of patients care. They will demonstrate safe clinical decision-making, ensuring that they act within their professional boundaries at all times. The post holder will work collaboratively with the multi-disciplinary practice teams and the wider Integrated Care team to meet the needs of patients and help to avoid unnecessary admissions to hospital. The post holder will provide support to conduct clinical audit, research, the development of clinical guidelines, procedures and protocols. Key Responsibilities Clinical Practice Provide direct consultation to patients in their own home, using decision making and problem solving skills to make differential diagnoses and ensure clinical practice is effective, evidence based and to a high standard. Agree management plans, in liaison with families, in accordance with agreed practice, local and national guidance. Demonstrates knowledge of the patho-physiology of acute and chronic conditions commonly seen in frail elderly patients. Initiates appropriate diagnostic requests and therapeutic interventions, with attention to safety, cost, invasiveness, accessibility and efficacy. Evaluates the patients condition and response to therapeutic interventions and modifies the plan of care accordingly. As an independent prescriber, prescribe safe, effective and appropriate medication for acute presentations, in accordance with current legislative framework. Help to develop clinical standards through policies and protocols that demonstrate evidence based practice and respond to patient needs. Ensure accurate contemporaneous clinical records are maintained on the clinical systems, including the use of templates and alerts where necessary. Nurse Management and Practice Administration Act as a point of liaison with other nurses across community services and the practice; act as mentor and clinical supervisor, supporting CPD. Plan and prioritise own clinical workload. Ensure adherence to the Drug Formulary, including the wound care formulary. Participate in Practice meetings, reporting progress as required and liaising with other members of the local ICO team. Health and Safety Ensure policies are understood and observed and that procedures are followed. Ensure that appropriate use of equipment and facilities and ensure the environment is maintained in good order. Take the necessary precautions to safeguard the welfare and safety of oneself and others in accordance with the Health and Safety Legislation. Ensure reporting systems are adhered to and promote a no blame culture. Undertake appropriate training as the Partners see fit.
